摘要 目的探讨缺血性脑卒中(IS)患者外周血及模型大鼠脑组织、脾脏中miR-199a-5p和膜微囊蛋白(Cav)-1的表达水平及临床意义。方法初发IS患者(发病24 h内)外周血标本24例,同时收集在体检中心体检的健康人外周血标本22例。Ficoll淋巴细胞分离液分离外周血单个核细胞(PBMC)。荧光定量PCR检测PBMC中miR-199a-5p和Cav-1的表达水平。采用改良Zea Longa线栓法构建SD大鼠大脑中动脉梗死(MCAO)模型。再灌注24 h后进行神经功能评分,收集脑皮质和脾脏组织。荧光定量PCR检测脑皮质和脾脏miR-199a-5p和Cav-1表达水平。结果 IS患者外周血PBMC中miR-199a-5p表达明显升高(P<0.001),Cav-1 mRNA表达显著下降(P<0.05)。与假手术组比较,MCAO模型组脑皮质和脾脏miR-199a-5p明显升高(P<0.001,P<0.01),Cav-1 mRNA表达显著下降(P<0.001,P<0.01)。结论 miR-199a-5p和Cav-1可能为IS患者重要的炎症指标。 Objective To investigate the levels of miR-199 a-5 p and Caveolin-1 in peripheral blood of ischemic stroke(IS) patients and in brain and spleen tissues of rat IS models and their clinical value. Methods A total of 24 patients with first-ever acute ischemic stroke patients within 24 h were selected. 22 health persons in the physical examination center were chosen as control group. Blood samples from the two groups were collected. Mononuclear cells were isolated using Ficoll lymphocytes separation medium following manufacturer′s instructions. Quantitative PCR(qPCR) was used to analyze the expressions of miR-199 a-5 p and Caveolin-1 in PBMC. Health adult male Sprague-Dawley(SD) rats were subjected to middle cerebral artery occlusion(MCAO). After 24 h MCAO, the neurological function scores were evaluated. qPCR was used to analyze the expressions of miR-199 a-5 p and Caveolin-1 in infarction cortex and spleen tissue. Results The variables of sex, age, fasting glucose, triglyceride, total cholesterol, high-density lipoprotein cholesterol, and low-density lipoprotein cholesterol between IS and control groups all showed no differences(all P>0.05). The level of miR-199 a-5 p in PBMC of IS group was significantly higher than that in control group(P<0.001). In contrast, the expression of Caveolin-1 mRNA was remarkably decreased in PBMC of IS group(P<0.05). Compare with those of sham group, the expressions of miR-199 a-5 p were significantly increased in the cortex and spleen tissues of MCAO rats(P<0.001, P<0.01). However, the expression of Caveolin-1 mRNA were remarkably decreased(P<0.001, P<0.01).Conclusions MiR-199 a-5 p and Caveolin-1 might be important inflammation markers of acute ischemic stroke.
