摘要
目的:观察体外膈肌起搏对脑卒中后气管切开患者拔管时间及肺部感染的疗效.方法:选择2017年1月-2018年7月我科收治脑卒中后气管切开患者50例,随机分为观察组和对照组各25例,所有患者均行常规康复治疗,对照组行传统呼吸训练及机械辅助排痰,观察组在此基础上联合应用体外膈肌起搏器(EDP)治疗.2组患者治疗60天后对其拔管时间、拔管率及肺部感染控制情况进行比较.结果:治疗60天后,观察组平均拔管时间较对照组缩短且拔管率明显提高(均P<0.05),观察组肺部感染有效率比对照组提高(P<0.05).结论:EDP应用于脑卒中后气管切开患者,可以缩短拔管时间,提高肺部感染疗效.
Objective: To determine the effect of external diaphragm pacemaker for the extubation time and pulmonary infection in tracheotomized patients after stroke. Methods: Included in this study were 50 tracheotomized patients after stroke treated in our department between January 2017 and July 2018. These patients were randomly assigned to experimental group( n =25) and control group( n = 25 ). All the patients received routine rehabilitation. In addition, the control group was subjected to traditional respiratory training and mechanical assisted sputum drainage, and the experimental group was treated with EDP on the basis of control group. After 60 days of treatment, the time and rate of extubation and the effect of pulmonary infection were compared in the two groups. Results: At 60 days after treatment ,the time of extubation was shorterand the rate of extubation was significantly higher in the experimental group than that in the control group( all P <0. 05 ). On day 60, the effective rate of pulmonary infection was higher in the experimental group than that in the control group( P <0. 05). Conclusion: EDP may shorten the time of extubation and improve the curative effect of pulmonary infection in tracheotomized patients after stroke.
