嵴顶入路微分区法提升倾斜上颌窦底的效果

摘要 目的评估嵴顶入路微分区法提升倾斜上颌窦底术的临床效果。方法选择上颌后牙区可用骨量不足且窦底倾斜的种植患者30例,随机分为两组,观察组采用嵴顶入路微分区提升术,对照组采用侧壁开窗入路上颌窦底提升术,在完成窦膜的提升和骨移植材料的置入后,同期植入种植体,经过6个月骨结合后完成上部修复。记录并分析术前和术后的窦底提升高度以及术后疼痛指数。结果 28例患者完成骨结合及最终修复,负重正常。观察组在倾斜上颌窦底的近中、正中和远中的提升高度分别为(4.41±0.61)、(5.45±0.40)和(5.74±0.27) mm,同时具有较低的疼痛指数。结论嵴顶入路微分区法提升倾斜上颌窦底术具有较好的临床效果。 Objective To evaluate the clinical efficacy of differential area method with ridge approach on elevating inclined maxillary sinus floor.Methods 30 patients with inclined maxillary sinus floor need bone augmentation were randomly divided into two groups,the experimental group(differential area method with ridge approach)and control group(lateral window approach).After the completion of elevation of sinus membrane with bone graft material,implants were inserted at the same time.The final restorations were delivered after six months for osseointegration.The preoperative and postoperative heights of sinus floor were recorded and analyzed;so was the postoperative pain index.Results Twenty-eight patients completed osseointegration and final restoration,with normal loading.In the experimental group,the elevations in the proximal,median and distal part of sinus floor were 4.41±0.61 mm,5.45±0.40 mm and 5.74±0.27 mm,respectively,with lower pain index value.Conclusion The differential area method with ridge approach to elevate the inclined maxillary sinus bottom has good clinical efficacy.
