食管癌患者心理一致感的现状及影响因素分析

Sense of coherence and associated factors in patients with esophageal cancer

目的分析食管癌患者的心理一致感现状及其影响因素,为临床制定针对性措施提供参考。方法采用一般情况调查、生活定向测验修订版量表、Herth希望量表、六项目感恩问卷、Hearland宽恕量表和心理一致感量表对240例食管癌患者进行问卷调查。结果食管癌患者心理一致感得分为(53.97±14.03)分,食管癌患者感恩、希望、乐观、宽恕得分与心理一致感总分以及各维度得分呈正相关(P<0.001)。多元线性回归结果表明肿块位置、化疗情况、希望、宽恕、乐观是食管癌患者心理一致感的主要影响因素(B=0.117～1.799,P<0.05)。结论食管癌患者心理一致感水平相对处于低水平且与肿块位置、化疗情况、希望、宽恕、乐观水平有关,医护人员应制定针对性的心理干预和健康教育,注重挖掘和发挥患者自身具有的积极心理品质的重要作用,以促进其心理健康。

Objective To investigate the level and associated factors of sense of coherence(SOC) in patients with esophageal cancer, and provides references for the clinical development of targeted measures. Methods Totally 240 esophageal cancer patients were investigated with Demographic Data Questionnaire, LOT-R(Revised Life Orientation Test), HHI(Herth Hope Inder), GQ-6(the Gratitude Questionnaire-6), HFS(Hearthland Forgiveness Scale), SOC-13(sense of coherence). Results The total scores of sense of coherence was 53.97±14.03. The scores of GQ-6, HHI, LOT-R and HFS in esophageal cancer patients were significantly positively correlated with the total score of sense of coherence and the scores of all dimensions(P<0.001). Multiple linear regression results showed that tumor location, chemotherapy, the score of the GQ-6, HHI, LOT-R and HFS were the main influencing factors of the sense of coherence(B=0.117-1.799, P<0.05). Conclusion Patients with esophageal cancer have a relatively low level of SOC, and it is correlated with the location of the tumor, chemotherapy, hope, forgiveness, and optimism. The patients should be given with targeted health education.
