加味八珍汤在晚期胃癌患者化疗中的应用研究

Effects of the modified bazhen tang in the patients with advanced gastric cancer during chemotherapy

目的研究对接受多西他赛、顺铂、氟尿嘧啶(DCF)化疗方案治疗的晚期胃癌患者应用中药是否具有增效减毒的作用。方法以2014年1月-12月住院的56例晚期胃癌患者为对象,将患者随机分为单纯化疗组(对照组)与中药联合组(治疗组)。对照组患者采用DCF化疗方案治疗,治疗组在化疗基础上联用八珍汤为主方治疗。比较两组患者近期疗效、远期疗效及化疗过程中出现的与化疗相关的不良反应情况。结果治疗组疾病控制率(DCR)为57. 1%,对照组为53. 6%,两组DCR比较差异无统计学意义(P=0. 592,P> 0. 05)。治疗组中患者中位无疾病进展时间(MPFS)为14个月,对照组为12个月,两组MPFS比较差异无统计学意义(P> 0. 05)。对照组患者白细胞减少、恶心、呕吐发生率明显高于治疗组(P <0. 05),差异具有统计学意义,而在脱发、肝肾毒性等不良反应方面,两组差异无统计学意义(P> 0. 05)。结论加味八珍汤对接受化疗的晚期胃癌患者,可以降低骨髓抑制发生率,减轻恶心呕吐症状。

Objective To explore whether Chinese herbal medicine increases the effects and reduces toxin in the patients with advanced gastric cancer during the chemotherapy with docetaxel+cis-platinum+fluorouracil(DCF).Methods From January to December2014,56 inpatients with advanced gastric cancer were collected as the subjects and randomized into a simple chemotherapy group(control group)and a Chinese herbal medicine combination group(treatment group).In the control group,the DCF chemotherapy was adopted.In the treatment group,on the base of the chemotherapy,bazhen tang was applied accordingly.The short-term therapeutic effects,the long-term therapeutic effects as well as the chemotherapy-related adverse reactions were compared between the two groups.Results The disease control rate was 57.1% in the treatment group and was 53.6% in the control group,without significant difference in comparison( P = 0.592 , P >0.05).The medial progress free survival(MPFS)was14 months in the treatment group and was12 months in the control group,without significant difference in comparison( P >0.05).The incidence rates of leucopenia,nausea and vomiting in the control group were higher obviously than the treatment group,indicating the significant differences(P<0.05 ).The differences were not significant in the adverse reactions such as hair loss and hepatic and renal toxicity between the two groups( P >0.05).Conclusion The modified bazhen tang reduces the incidence rate of myelosuppression and alleviates nausea and vomiting in the patients with advanced gastric cancer during chemotherapy.
