进展性卒中与H型高血压的相关性研究

Study on the Correlation between Progressive Stroke and H-type Hypertension

目的:探讨卒中进展与H型高血压的关系。方法:选取笔者所在医院神经内科2015年1月-2017年12月住院的缺血性卒中患者194例,所有患者入院时间为发病后24 h内,按照患者72 h内病情有无发生进展分为两组(进展组和非进展组),回顾其各自的临床基本资料、血清同型半胱氨酸(Homocysteine,Hcy)水平,分析探讨进展性卒中与H型高血压的关系。结果:194例患者有57例(29.4%)发生进展。进展组患者年龄、糖尿病、高脂血症、吸烟史、酗酒史、卒中家族史等的发生率,与无进展组比较,差异无统计学意义(P>0.05);H型高血压组、HHcy组的卒中患者进展发生率明显高于单纯高血压组及正常组,差异有统计学意义(P<0.05)。结论:H型高血压、HHcy都是进展性卒中的独立的危险因素,加强对这些因素的管理对于进展性脑卒中的防治及预后具有十分重要的意义。

Objective:To investigate the relationship between progressive stroke and H-type hypertension.Method:A total of 194 patients with ischemic stroke admitted to the department of neurology of the hospital from January 2015 to December 2017 were selected.All patients were hospitalized within 24 hours after onset.They were divided into two groups(the progressive group and the non-progressive group) according to their condition within 72 hours.Their clinical basic data and serum homocysteine levels were reviewed to explore the relationship between progressive apoplexy and H-type hypertension.Result:There were 57 cases(29.4%) with progression in 194 patients.The incidence of progressive stroke in stroke patients,such as age,diabetes,hyperlipidemia,smoking history, alcohol history and family history of stroke,were compared with the non-progressive group,the differences were not statistically significant(P>0.05).The incidence of stroke progression in the H-type hypertension group and HHcy group were significantly higher than those in the simple hypertension group and the normal group, the differences were statistically significant(P<0.05).Conclusion:H-type hypertension and HHcy are independent risk factors for progressive stroke.Strengthening the management of these factors have great significance for the prevention,treatment and prognosis of progressive stroke.
