摘要
目的探讨中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、白介素-6(IL-6)和胱抑素C(Cys-C)在糖尿病肾病早期诊断中的价值。方法选取2017年3月~2017年9月于我院内分泌科就诊的2型糖尿病患者136例作为研究对象。按24h尿微量白蛋白排泄率(UAER)分为三组,单纯糖尿病(DM)组(n=48),早期糖尿病肾病(DN)组(n=45)和糖尿病肾病(DN)组(n=43)。同时招募了40名非糖尿病受试者作为对照组。采用全自动生化分析仪测定UREA、Scr和Cys-C,应用定量免疫层析法和上转发光法对IL-6和NGAL进行测定。结果单纯DM组UREA、Scr、Cys-C、IL-6和NGAL结果和对照组比较无显著差异(P>0.05)。早期DN组UREA、Scr结果和对照组及单纯DM组相比无显著差异(P>0.05),但早期DN组NGAL、IL-6和Cys-C结果高于单纯DM组(P<0.05)。DN组和单纯DM组、早期DN血清NGAL、IL-6和Cys-C的联合检测对于2型糖尿病肾病早期诊断具有一定价值。
Objective This study is designed to investigate the values of neutropil gelatinase-associated lipocalin (NGAL), Interleukin-6(IL-6)and Cystatin C(Cys-C)in the early detection of diabetic nephropathy. Methods A total of 136 patients with type 2 diabetes who sought medical treatment in the Department of Endocrinology of our hospital from March 2017 to September 2017 were selected. The participants were divided by 24h urinary albumin excretion rate (UAER) into three groups: simple DM group (n = 48), early DN group (n = 45) and DN group (n = 43). Furthermore, 40 non-diabetic subjects were recruited as the control group. UREA, Scr and Cys-C were measured by an automatic biochemical analyzer, and IL-6 and NGAL were measured by quantitative immunochromatography and upconversion luminescence. Results There was no significant difference in UREA, Scr, Cys-C, IL-6 and NGAL values between the simple DM group and the control group (P>0.05). The early DN group was not significantly different from the control group and the simple DM group in UREA and Scr values, but had higher NGAL, IL-6 and Cys-C values than the simple DM group(P<0.05). There were significant differences between the DN group and the simple DM group, the early DN group and the control group (P<0.05). Conclusions Joint detection of NGAL, IL-6 and Cys-C has some value for early detection of type 2 diabetic nephropathy.
