外固定支架术对桡骨远端关节内骨折患者腕关节功能的影响

Effect of external fixator on wrist function in patients with intra-articular distal radius fractures

目的探讨外固定支架术对桡骨远端关节内骨折患者腕关节功能的影响。方法选取本院2015年12月至2017年12月间收治的桡骨远端关节内骨折患者64例,随机分为对照组(32例)和观察组(32例)。对照组采用传统切开内固定术治疗,观察组采用外固定支架术治疗。比较两组临床疗效及术后腕关节功能恢复情况。结果两组治疗前尺偏角、掌倾角,治疗后骨折愈合时间比较差异均无统计学意义;观察组手术时间、术中出血量均明显少于对照组(P<0.05);观察组治疗后尺偏角明显低于对照组,掌倾角明显高于对照组(P<0.05);观察组治疗后腕关节功能优良率96.88%(31/32)明显高于对照组81.25%(26/32),差异具有统计学意义(P<0.05)。结论外固定支架术可显著缩短桡骨远端关节内骨折患者手术时间、减少术中出血量,改善尺偏角、掌倾角及腕关节活动功能,临床疗效显著,值得推广应用。

Objective To investigate the clinical efficacy of external fixator in the treatment of intra-articular distal radius fractures and its effect on wrist joint function. Methods 64 cases of distal radius intra-articular fractures treated in our hospital from December 2015 to December 2017 were selected as the research subjects. They were randomly divided into the control group(32 cases) and the observation group(32 cases). The control group was treated by traditional incision and internal fixation, while the observation group was treated with external fixator. The clinical efficacy and postoperative recovery of wrist function were compared between the two groups. Results There was no significant difference between the two groups before treatment. There was no significant difference in the healing time of fracture after treatment. The amount of bleeding and the operation time of the observation group were less than that of the control group(P<0.05). The angle of the ulnar angle in the observation group was significantly lower than that of the control group, and the palmar angle was significantly higher than that of the control group(P<0.05). The excellent and good rate of wrist function in the observation group after treatment was 96.88%(31/32), which was significantly higher than that in the control group 81.25%(26/32), and the difference was statistically significant(P<0.05). Conclusion External fixation can shorten the operation time, reduce the amount of bleeding, improve the angle of the ulnar, the angle of the palmar and wrist joint, and the clinical effect is remarkable. It is worth popularizing.
