摘要 目的探讨间接免疫荧光方法研究EBV感染类型与免疫功能关系。方法选择2016年5月至2018年7月就诊于泉州市儿童医院&妇幼保健院怀疑EB病毒感染的住院患儿166例,间接免疫荧光方法联合检测EB病毒四种抗体及CA-IgG亲和力;同时检测患儿免疫功能:外周血白细胞WBC、淋巴细胞LB、T淋巴细胞、B淋巴细胞、NK细胞和T细胞亚群CD4^+T、CD8^+T;外周血体液免疫球蛋白IgA、IgM、IgG和补体C3、C4。结果急性感染组EBV衣壳抗原CA-IgM阳性率只有9.75%,早期抗原抗体EA-IgG阳性率比较高(36.58%)。急性感染组B淋巴细胞、CD4^+T%、CD4^+T/CD8^+T这三个指标比无感染组低(P值为0.025,0.003,0.01)。急性感染组CD8+T%值比无感染组高(P=0.005)。既往感染组IgG和IgA比复发感染和无感染组高(P<0.05)。结论分析不同感染类型免疫功能的差异,有助于临床诊治。 Objective To investigate the relationship between EBV infection type determined by indirect immunofluorescence and immune function. Methods A total of 166 children with suspected EBV infection who were hospitalized in Children & women Hospital of Quanzhou from May2016 to July 2018 were enrolled in this study. Indirect immunofluorescence method was used to detect four antibodies and affinities of EBV virus,meanwhile, immune function indicators such as peripheral blood leukocyte WBC, lymphocyte LB, T lymphocytes, B lymphocytes, NK cells,T cell subgroup CD4^+T, CD8^+T count and peripheral blood humoral immunoglobulin IgA, IgM, IgG and complement C3, C4 were detected. Results The positive rate of EBV capsid antigen CA-IgM in acute infection group was only 9.75% but the positive rate of early antigen antibody EA-IgG was higher(36.58%). The values of B lymphocytes, CD4^+T%, CD4^+/CD8^+ in the acute infection group were lower than the non-infected group(P value was 0.025, 0.003, 0.01). The acute infection group had higher CD8^+T% than the non-infected group(P= 0.005). The values of IgG and IgA were higher in the past infected group than that in the recurrent infected group and non-infected groups(P<0.05). Conclusion Analysis of the differences in immune function of different infection types is helpful for clinical diagnosis and treatment.
