摘要
目的探讨血液灌流技术(HP)应用于重症有机磷农药中毒(AOPP)患者抢救疗效。方法回顾性分析2010年4月至2018年4月本院收治的AOPP患者临床资料,将采用常规治疗的患者纳入常规治疗组(n=20),将在常规治疗基础上给予血液灌流治疗的患者纳入血液灌流组(n=35),比较两组一般治疗效果及肝肾功能变化情况,记录并发症。结果血液灌流组治疗有效率97.14%显著高于常规治疗组75.00%,差异有统计学意义(P<0.05);血液灌流组CHE恢复时间、阿托品用量、住院时间均短于常规治疗组,差异比较有统计学意义(P<0.05);治疗后两组ALT、AST均降低,且血液灌流组治疗后以上指标均显著低于常规治疗组,差异有统计学意义(P<0.05);血液灌流组并发症发生率2.86%显著低于常规治疗组20.00%,差异有统计学意义(P<0.05)。结论 HP应用于AOPP患者抢救疗效较佳,可显著降低对肝功能的损伤,安全有效,值得在临床上推广。
Objective To investigate the curative effect of hemoperfusion(HP) technique on rescue for patients with severe acute organophosphorus pesticide poisoning(AOPP). Methods The clinical data of AOPP patients admitted to the hospital from April 2010 to April 2018 were retrospectively analyzed. Patients undergoing routine therapy were included in the routine treatment group(n=20). Patients undergoing HP on the basis of routine treatment were included in HP group(n=35). The general curative effects and changes of liver and kidney function were compared between the two groups. The complications were recorded. Results The response rate of treatment in HP group was significantly higher than that of the routine treatment group(97.14% vs 75.00%)(P<0.05). The recovery time of CHE, atropine dosage and hospitalization time in HP group were shorter than those in the routine treatment group(P<0.05). ALT and AST were decreased in both groups after treatment. The above indexes in HP group were significantly lower than those in the routine treatment group(P<0.05). The incidence of complication in HP group was significantly lower than that in the routine treatment group(2.86% vs 20.00%)(P<0.05). Conclusion The curative effect of HP is relatively better on rescue for AOPP patients, which can significantly reduce the damage to liver function. It is safe and effective.
