256层iCTA在心肌病合并冠状动脉狭窄及左心功能评估中的价值研究

A 256-layer iCTA study on cardiomyopathy with coronary artery stenosis and left heart function

目的:研究256层螺旋CT血管造影(iCTA)应用于心肌病合并冠状动脉狭窄及左心功能评估中的效果。方法:接受冠状动脉血管造影(CTA)检查的心肌病患者70例为观察对象。分别对所有患者进行256层iCTA检查,常规冠状动脉血管造影(CTA)检查以及超声心动图检查。并对比256层iCTA与常规CTA检查的冠状动脉狭窄程度情况,256层iCTA与超声心动图检查的各项左心功能指标水平。结果:256层iCTA与常规CTA检查在冠状动脉轻度狭窄、中度狭窄、重度狭窄人数占比方面对比,均无统计学差异(均P>0.05)。256层iCTA与超声心动图检查在LVEF、LVSV、LVESV、LVEDV水平方面对比,均无统计学差异(均P>0.05)。结论:256层iCTA应用于心肌病合并冠状动脉狭窄诊断中的效果与CTA检查高度一致,而应用于左心功能评估中的效果与超声心动图无明显差异。在临床工作中可通过对心肌病患者进行256层iCTA检查,从而明确其冠状动脉狭窄情况以及左心功能。

Objective:To investigate and analyze the effect of 256-slice spiral CT angiography(iCTA)in the evaluation of coronary artery stenosis and left heart function in patients with cardiomyopathy.Methods:70 patients with cardiomyopathy who received coronary artery CTA examination were recorded as the objects of observation.All patients underwent 256-layer iCTA,conventional coronary angiography(CTA)and echocardiography,respectively.The degree of coronary artery stenosis detected by 256-layer iCTA and conventional CTA was compared,and the left heart function indexes detected by 256-layer iCTA and echocardiography were compared.Results:The proportion of patients with mild,moderate and severe coronary artery stenosis in the 256-layer iCTA and conventional CTA examination was not significant(all P>0.05).There was no significant difference in LVEF,LVSV,LVESV and LVEDV levels between the 256-layer iCTA and echocardiography(all P>0.05).Conclusion:The effect of 256-layer iCTA in the diagnosis of cardiomyopathy complicated with coronary artery stenosis is highly consistent with that of CTA examination,while the effect of left heart function evaluation is not significantly different from that of echocardiography.Which can conduct 256-layer iCTA examination on patients with cardiomyopathy in clinical work,so as to clarify their coronary artery stenosis and left heart function.
