乙肝肝硬化患者中微小RNA-21、Th9的表达及其对肠道屏障功能调控机制的影响

Expression of miR-21 and Th9 and its regulation mechanism on intestinal barrier function in patients with hepatitis B cirrhosis

目的探讨乙肝肝硬化患者中微小RNA-21(miR-21)、辅助性T淋巴细胞9(Th9)的表达及其对肠道屏障功能调控机制的影响。方法选择乙肝肝硬化患者144例作为肝硬化组,同期选择体格检查健康者144例作为对照组,记录两组入选者的常规检测指标,测定血液中miR-21、Th9、二胺氧化酶(DAO)、内毒素(ET)、D-乳酸(D-lac)表达水平并进行相关性分析。结果肝硬化组的血清丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、总胆红素(TBIL)含量都高于对照组,肝硬化组的miR-21相对表达量与Th9含量都高于对照组,肝硬化组的血清ET、DAO与D-lac值高于对照组(P<0.05)。在肝硬化组中,直线相关分析显示血清ET、DAO、D-lac与ALT、TBIL、AST、miR-21、Th9都呈现显著正相关性(P <0.05)。结论乙肝肝硬化患者中miR-21、Th9都呈现高表达状况,也表现为肠道屏障功能紊乱状态,两者可互相形成,形成恶性循环,从而参与乙肝肝硬化的发生与发展。

Objective Investigation of the expression of miR-21 and helper T lymphocytes(Th9)in patients with hepatitis B cirrhosis and its effect on the regulation of intestinal barrier function.Methods 144 patients with hepatitis B cirrhosis were selected as the cirrhosis group,and 144 healthy subjects were selected as the control group.The routine detection indexes of the two groups were recorded,including the expression levels of miR-21,Th9,DAO,ET and D-lac in the blood and the correlation analysis was done.Results The levels of ALT,TBIL and AST in serum in the cirrhosis group were higher than those in the control group.The relative expression of miR-21 and Th9 in the cirrhosis group were higher than those in the control group.The value of ET,DAO and D-lac of the serum in the cirrhosis group was higher than those in the control group(P<0.05).In the cirrhosis group,linear correlation analysis showed that the value of ET,DAO,D-lac in the serum showed a significant positive correlation with the value of ALT,TBIL,AST,miR-21,and Th9(P<0.05).Conclusions miR-21 and Th9 in patients with hepatitis B cirrhosis have high expression status,and also manifest as intestinal barrier dysfunction.These two factors can be derived from each other and form a vicious circle,thus they participate in the occurrence and development of hepatitis B cirrhosis.
