不同切口超声乳化术联合小梁切除术治疗白内障合并青光眼疗效研究

Therapeutic Effect of Different Incision Phacoemulsification Combined with Trabeculectomy for Cataract with Glaucoma

目的探究不同切口超声乳化术联合小梁切除术治疗白内障合并青光眼疗效。方法采用随机数字表法将方便选取该院2017年5月—2018年5月入院行白内障、青光眼手术治疗的90例白内障合并青光眼患者分成观察组与对照组各45例,观察组采用双切口超声乳化+小梁切除术,对照组采用小切口超声乳化+小梁切除术,探讨实施不同治疗的疗效。结果观察组治疗后眼内压显著比对照组高,差异有统计学意义(t=15.957,P<0.05),裸眼视力显著比对照组高,差异有统计学意义(t=12.883,P<0.05),术后观察组功能型滤过泡形成率(95.56%)显著比对照组(77.78%)高,差异有统计学意义(χ~2=6.154,P<0.05),观察组并发症发生率(6.67%)与对照组相比(26.67%)显著较低,差异有统计学意义(χ~2=4.406,P<0.05)。结论将双切口超声乳化、小梁切除应用于白内障合并青光眼治疗后,患者术后并发症发生率低,更为安全可靠,且眼内压、视力改善理想,功能型滤过泡形成率高,此方法值得应用与推广。

Objective To investigate the efficacy of different incision phacoemulsification combined with trabeculectomy in the treatment of cataract with glaucoma. Methods A randomized digital table method was used to divide 90 patients with cataract and glaucoma who underwent cataract surgery and glaucoma surgery into 45 patients in the observation group and the control group from May 2017 to May 2018. The observation group was treated with double-incision phacoemulsification and trabeculectomy. The control group used small incision phacoemulsification + trabeculectomy to explore the efficacy of different treatments. Results The intraocular pressure of the observation group was significantly higher than that of the control group,the difference was statistically significant (t =15.957, P <0.05). The visual acuity of the naked eye was significantly higher than that of the control group,the difference was statistically significant (t =12.883, P <0.05). The functional bleb formation of the postoperative observation group was (95.56%) was significantly higher than that of the control group (77.78%),the difference was statistically significant (χ^2=6.154, P<0.05). The incidence of complications in the observation group (6.67%) was significantly lower than that of the control group (26.67%),the difference wasstatistically significant (χ^2=4.406, P<0.05). Conclusion After double-incision phacoemulsification and trabeculectomy for cataract with glaucoma treatment, the postoperative complication rate is low, safer and more reliable, and the intraocular pressure and visual acuity are better, and the functional filtration bleb formation rate is high. This method is worthy of application and promotion.
