前置胎盘剖宫产术中球囊与非球囊治疗产后出血的应用对比

Comparative study of balloon and non-balloon treatment for hemorrhage in patients with placenta previa after cesarean section

目的:探讨前置胎盘剖宫产术中Bakri球囊和非球囊方案治疗产后出血的有效性。方法:回顾性分析医院收治的因前置胎盘行剖宫产并发生产后出血的91例产妇,89例纳入本研究,分球囊组(57例)和非球囊组(32例)。观察两组子宫切除率、失血量、输血量、二次手术及是否入住ICU情况。结果:球囊组91.22%的患者单用球囊成功实现了止血,较非球囊组出血量显著降低(P<0.001),手术及住院时间显著缩短(P<0.001)。相关性分析结果显示,球囊治疗成功率与宫缩(P=0.037)和手术时间(P=0.039)密切相关。结论:在前置胎盘患者发生产后出血时,应用Bakri球囊可有效止血,是一种可广泛推广应用的经济、有效的方法。

Objective:To compare the effectiveness of the Bakri balloon and non-balloon regimens in patients with placenta previa(PP) for the treatment of postpartum hemorrhage(PPH) in cesarean section(CS). Methods:A retrospective analysis of 91 cases of PPH occurred in patients with PP after CS in the department of obstetrics and gynecology of hospital,of which 89 were included in the study, which were divided into balloon group(57 cases) and non-balloon group(32 cases). The main observations were prevention of hysterectomy, blood loss, blood transfusions, secondary surgery, and admission to ICU wards. Results: In the balloon group, 91.22% of patients achieved hemostasis with a single balloon, and the amount of hemorrhage was significantly lower than that in the non-balloon group(P<0.001), the operation time and the length of stay was significantly shortened(P<0.001).The correlation analysis showed that balloon treatment was closely related to contractions(P=0.037) and operation time(P=0.039). Conclusion: When PPH occurs in patients with PP, the use of Bakri balloon can effectively treat PPH. It is an economical and effective strategy that can be widely applied.
