腹腔镜手术联合GnRH-a对卵巢子宫内膜异位症患者血清标志物水平及卵巢储备功能的影响

Effect of laparoscopic surgery combined with GnRH-a on serum markers and ovarian reserve function in patients with ovarian endometriosis

目的:探讨卵巢子宫内膜异位症采用腹腔镜手术联合促性腺激素释放激素类似物(GnRH-a)治疗的效果。方法:选取某院收治的卵巢子宫内膜异位症患者104例,按单盲法分为2组,每组各52例。对照组采用腹腔镜手术治疗,在此基础上,观察组采用GnRH-a治疗。治疗6个月后,比较2组患者临床疗效、复发率、卵巢储备功能及血清标志物水平。结果:观察组治疗总有效率高于对照组,复发率低于对照组,2组比较差异有统计学意义(P<0.05);治疗后,观察组血清促黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E_2)、血清糖类抗原125(CA125)、糖类抗原72-4(CA72-4)、人附睾蛋白4(HE4)水平均低于对照组,差异具有统计学意义(P<0.05)。结论:卵巢子宫内膜异位症采用腹腔镜手术联合GnRH-a治疗可提升临床疗效,改善卵巢储备功能,降低疾病复发率及血清标志物水平。

Objective :To investigate the effects of laparoscopic surgery combined with gonadotropin releasing hormone analogue (GnRH-a) in treatment of ovarian endometriosis. Methods :104 patients with ovarian endometriosis admitted to hospital were selected and were divided into two groups by single blind method, with 52 cases in each.The control group was given laparoscopic surgery, while the observation group was treated with GnRH-a based on the program of the control group. 6 months afteRthe treatment, clinical efficacy, recurrence rate, ovarian reserve function, and serum markers were compared between the two groups. Results :Compared with control group, the total effective rate of the observation group was higher, while the recurrence rate of the observation group was loweR( P <0.05). AfteRthe treatment, the levels of luteinizing hormone (LH), follicle-stimulating hormone (FSH), estradiol (E 2 ), serum carbohydrate antigen 125 (CA125), carbohydrate antigen 72-4 (CA72-4), and human epididymis protein 4 (HE4) in the observation group were loweRthan those in the control group ( P <0.05). Conclusion :Laparoscopic surgery combined with GnRH-a in treatment of ovarian endometriosis can increase clinical efficacy, decrease recurrence rate, improve ovarian reserve function, and reduce levels of serum markers.
