连续性血液净化治疗危重患儿急性肾损伤的疗效及对肾功能、血清炎症因子水平的影响

Effects of CBP in treatment of AKI in critically ill children and influence on renal function and serum inflammatory factors

目的:探讨连续性血液净化(CBP)治疗危重急性肾损伤(AKI)患儿的效果及对肾功能、血清炎症因子水平的影响。方法:选取2016年3月—2018年3月某院收治的AKI危重患儿94例,采用随机数表法分为观察组和对照组,每组各47例,对照组给予间歇性血液透析(IHD)治疗,观察组给予连续性血液净化治疗,比较2组治疗效果及肾功能、血清炎症因子水平的变化。结果:治疗有效率观察组(95.74%)高于对照组(78.72%),差异有统计学意义(P<0.05);治疗后1天、治疗后1周2组BUN,Scr,IL-6,TNF-α水平均明显降低,且观察组低于对照组,差异有统计学意义(P<0.05)。结论:CBP治疗危重患儿AKI效果较佳,可显著改善患儿肾功能,降低机体炎症反应的发生,值得在临床上推广。

Objective :To investigate the effects of continuous blood purification (CBP) in treatment of acute kidney injury (AKI) in critically ill children and the influence on renal function and serum inflammatory factors. Methods :A total of 94 critically ill children with AKI admitted to hospital were selected and randomly divided into an observation group and a control group,47 cases in each.The control group was treated with intermittent hemodialysis(IHD) and the observation group was treated with CBP.Curative effect and changes of renal function and serum inflammatory factors were compared between the two groups. Results :The effective rate of treatment in the observation group was significantly higheRthan that in the control group (95.74% vs 78.72%)( P <0.05).Levels of BUN,Scr, IL-6 and TNF-α in both groups were significantly decreased at day 1 and one week afteRthe treatment. Each index in the observation group was significantly loweRthan that in the control group at the same time point( P <0.05). Conclusion :CBP is effective in the treatment of critically ill children with AKI.It can significantly improve the renal function and reduce the incidence of inflammatory response.
