自体颗粒脂肪移植在面部凹陷填充中的临床应用及远期疗效观察

Clinical Application and Long-term Effect of Autologous Fat Transplantation in Facial Plastic Surgery

目的:探讨自体颗粒脂肪移植在面部凹陷填充中的临床应用价值及远期效果。方法:选取笔者医院2014年6月-2016年6月收治的87例面部凹陷患者,均采用自体脂肪颗粒移植技术进行填充修复,供脂区主要选择在大腿外侧、臀部、下腹部等脂肪含量丰富部位。术后设定随访期1～2年,根据手术前后患者不同角度的面部照片对比情况初步评估手术修复效果,术后随访期间重点观察表情恢复、脂肪吸收情况以及并发症发生情况,并进行面部整形满意度调查。结果:本组87例患者随访8个月～2年,平均(14.6±4.8)个月,自体脂肪颗粒移植共注射115次,平均注射约1.3次。术后未出现血肿、局部硬结等并发症,随访3～6个月时脂肪吸收基本趋于稳定。截至随访末期,本组患者手术整形总有效率为97.70%(85/87),患者有面部丰满匀称、皮肤平整和表情恢复自然等变化。对整形效果、手术并发症和生活质量自我评估满意度分别为64.37%(56/87)、78.16%(68/87)、80.46%(70/87)。结论:自体颗粒脂肪移植具有脂肪吸收低、并发症少、移植填充效果显著等优点,远期疗效值得肯定,是一种安全、简单、可靠的面部凹陷填充方法。

Objective To investigate the clinical value and long-term effect of autologous fat transplantation in facial plastic surgery. Methods Eighty-seven patients with facial depression who were admitted to plastic surgery from June 2014 to June 2016 were repaired with autologous fat granule transplantation. The fat donor areas were mainly located in the fat-rich areas of the patients' thighs, buttocks and lower abdomen. Postoperative follow-up period of 1-2 years, according to the different angles of the patients before and after the operation of facial photographs, preliminary evaluation of the effect of surgical repair, postoperative follow-up focus on the observation of expression recovery, fat absorption and related complications, patients with facial plastic satisfaction survey. Results The patients were followed up for 8 months to 2 years, with an average of (14.6±4.8) months. Autologous fat granules were injected 115 times, with an average injection of about 1.3 times. There were no complications such as hematoma or local induration, and fat absorption tended to be stable at 3-6 months follow-up. Up to the end of follow-up, 97.70%(85/87) of the total effective rate of plastic surgery in this group, patients with facial plumpness and symmetry, skin flatness and expression of natural changes. The satisfaction rate of plastic effect, surgical complications and quality of life were 64.37%(56/87), 78.16%(68/87) and 80.46%(70/87). Conclusion Autologous fat granule transplantation has the advantages of low fat absorption, fewer complications and remarkable filling effect. It is a safe, simple and reliable facial plastic therapy.
