鼻内镜下泪前隐窝入路手术治疗KrouseⅢ期上颌窦内翻性乳头状瘤的疗效观察

Effect of intranasal endoscopic prelacrimal recess approach in the treatment of Krouse Ⅲ stage inverted papilloma in maxillary sinus

目的探讨鼻内镜下泪前隐窝入路治疗KrouseⅢ期上颌窦内翻性乳头状瘤的效果。方法选取诊断为KrouseⅢ期上颌窦内翻性乳头状瘤患者21例,随机分为泪前隐窝组(11例)和柯-陆手术组(10例),术后随访12～96个月。观察并分析两组患者的手术时间、住院时间、术后并发症和复发率。结果泪前隐窝组患者的住院时间短于柯-陆手术组,并发症发生率亦低于柯-陆手术组(P<0.05或0.01);而手术时间和肿瘤复发率两组差异无统计学意义(P>0.05)。结论鼻内镜下泪前隐窝入路手术是一种安全、微创、有效的治疗手段,可作为KrouseⅢ期上颌窦内翻性乳头状瘤的首选术式。

Objective To explore the effect of intranasal endoscopic prelacrimal recess approach in the treatment of Krouse stage Ⅲinverted papilloma in maxillary sinus.Methods A total of 21 patients with Krouse stage Ⅲ inverted papilloma of the maxillary sinus were randomly divided into two groups.One group (PLRA Group) had 11 patients that were treated with intranasal endoscopic prelacrimal recess approach while another group (Combination Group) had 10 patients that were treated with endoscopic technique combined with Caldwell-Luc approach.The 21 patients were subject to postoperative follow-up for 12-96 months.the operation time,hospital stay,postoperative complications and recurrence rate of the two groups were observed and compared.Result The PLRA Group had the hospital stay shorten than the Combination Group and the complication incidence lower than the Combination Group (P<0.05 or 0.01).There was no statistical difference between two groups of operative time and recurrence rate (P>0.05).Conclusion Intranasal endoscopic prelacrimal recess approach is a minimally invasive,safe and effective method for Krouse stage Ⅲ inverted papilloma of maxillary sinus and can be used as the first choice.
