脂蛋白相关磷脂酶A2和同型半胱氨酸与冠心病的相关性研究 被引量:8

摘要 目的探讨冠心病患者外周血脂蛋白相关磷脂酶A2(LP-PLA2)及同型半胱氨酸(Hcy)水平与冠心病的关系及临床应用价值。方法选择该院2016年7月至2017年12月行冠状动脉造影检查诊断为冠心病的患者338例,根据冠状动脉造影检查及其他辅助检查的结果,把冠心病患者分无症状性心肌缺血组(SMI组)45例,稳定型心绞痛组(SAP组)49例,不稳定型心绞痛组(UAP组)122例,非ST段抬高心肌梗死组(NSTEMI组)68例,ST段抬高型心肌梗死组(STEMI组)54例,另选择该院同期体检健康者93例为对照组,比较各组间血清LP-PLA2和Hcy水平的差异,分析LP-PLA2和Hcy水平与冠心病的相关性。结果各组患者高密度脂蛋白胆固醇(HDL-C)、Hcy及LP-PLA2等指标差异有统计学意义(P<0.05);随着冠心病的发展,Hcy、LP-PLA2水平逐渐升高,HDL-C水平逐渐降低,差异有统计学意义(P<0.05)。Logistic回归分析显示,LPPLA2、Hcy是冠心病发生的危险因素(Hcy OR=1.222,95%CI:1.072~1.393,P<0.05;LP-PLA2 OR=1.025,95%CI:1.019~1.031,P<0.05)。结论 Hcy、LP-PLA2在一定程度上可以反映冠心病的严重程度,可为冠心病的预测及早期诊治提供参考价值。 Objective By detecting the level of homocysteine(Hcy)and plasma lipoprotein associated phospholipase A2(LP-PLA2)in coronary heart disease(CHD)patients to explore their relationship with CHD and clinical application value.Methods A total of 338 cases of patients which diagnosed with coronary heart disease by coronary angiography in our hospital from Jul 2016 to Dec 2017 were collected.According to the results of coronary angiography and other auxiliary examinations,the CHD group was further divided into 5 groups of silent myocardial ischemia(SMI,45 cases),stable angina(SAP,49 cases),unstable angina(UAP,122 cases),non ST segment elevation myocardial infarction(NSTEMI,54 cases),ST segment elevation myocardial infarction(STEMI,68 cases).In addition,93 healthy people who had physical examination in our hospital during the same period were selected as the control group.LP-PLA2 and Hcy levels of each group were compared to analyze the correlation between LP-PLA2 and Hcy levels and CHD.Results There were statistically significant differences in HDL-C,Hcy and LP-PLA2 in each groups(P<0.05),With the development of coronary heart disease,Hcy,LP-PLA2 gradually increased and HDL-C gradually decreased(P<0.05).Logistic regression analysis showed that LP-PLA2 and Hcy were risk factors for the occurrence of coronary disease(Hcy OR=1.222,95%CI:1.072-1.393,P<0.05;LP-PLA2 OR=1.025,95%CI:1.019-1.031,P<0.05).Conclusion Hcy and LP-PLA2 can reflect the severity of coronary heart disease to some extent and provide a positive reference value for the prediction,early diagnosis and treatment of coronary heart disease.
