接触式A型超声与IOL Master检测人工晶状体屈光度精准度分析 被引量:1

摘要 目的对比分析接触式A型超声与IOL Master检测人工晶状体屈光度的精准度。方法本组共计45例(45只眼),其中24只眼植入RA人工晶状体(英国瑞纳公司生产的非球面人工晶状体,A型超声A常数为118. 0,IOL Master A常数为118. 5),21只眼植入ZMB00人工晶状体(强生全视公司生产的非球面衍射多焦点人工晶状体,A型超声A常数为118. 8,IOL Master A常数为119. 8)。分别使用A型超声及IOL Master测量。其中A型超声测量组使用接触式A型超声测量眼轴长度,角膜曲率计测量角膜曲率。而生物测量组使用IOL Master 500利用光学干涉仪原理一次完成眼轴长度及角膜曲率测量。两组均使用SRK/T公式计算人工晶状体屈光度。术后使用TOPCON视觉分析系统检测术后屈光状态,利用等效球镜法计算出术后屈光状态,并与超声测量组及生物测量组的目标屈光度进行对比,使用SPSS 12. 0统计学软件分析两组差异。结果植入的人工晶状体屈光度17. 5~23. 5 D,平均为(20. 92±1. 53) D。45只眼平均的术后屈光状态为(-0. 44±0. 87) D,A型超声测量组的目标屈光度为(-0. 41±0. 10) D,生物测量组目标屈光度为(-0. 22±0. 38) D。经t检验,45只眼术后屈光状态与两组目标屈光度之间无统计学意义(P> 0. 05)。结论 IOL Master虽然具有不接触角膜的优点,理论上测量更加准确,但是通过对比研究发现,两组无明显的统计学差异,说明A型超声测量仍然具有一定的参考价值。 Objective To compare the accuracy of contact type A ultrasound and IOL Master in the detection of intraocular lens diopter.Methods A total of 45 eyes,of which 24 eyes implanted with RA IOL(Aspheric intraocular lens by the British Rena company produced,A type ultrasonic A constant is 118.0,IOL Master A constant is 118.5),21 eyes of ZMB00 intraocular lens implantation(Johnson as the company's production of aspheric diffractive multifocal intraocular lens,A ultrasonic A constant is 118.8,IOL Master A constant is 119.8).A type ultrasound and IOL Master measurements were used respectively.In the A ultra measuring group contact type A ultrasound was used to measure the length of eye axis,and corneal curvature was measured by corneal curvature meter.In the biological measurement group,IOL Master 500 was used to measure eye axis length and corneal curvature at one time using the principle of optical interferometer.The diopter of the intraocular lens was calculated by SRK/T formula in both groups.TOPCON visual analysis of postoperative refractive status detection system was used,using spherical equivalent method to calculate the postoperative refractive status,and compared with the ultrasonic measurement group and biological measurement group target diopter,using SPSS 12 statistical software statistical analysis of the differences between the two groups.Results The intraocular lens diopter was 17.5D^23.5D,with an average of 20.92±1.53D.The average refractive state of the 45 eyes was-0.44±0.87D,and the target diopter of the A super measurement group was-0.41±0.10D,and the target diopter of the biological measurement group was-0.22±0.38.After t test,there was no statistical significance between the refractive index of the 45 eyes and the target diopter of the two groups(P>0.05).Conclusions Although IOL Master has the advantages of non-contact corneal,the measurement is more accurate in theory,but it is found that there is no significant statistical difference between the two groups,indicating that A type ultrasonic measurement still has a certain reference value.
