全身骨显像与SPECT/CT融合显像对单发骨转移瘤诊断效能比较 被引量:12

摘要 目的比较全身骨显像与SPECT/CT融合显像对单发骨转移瘤的诊断效能。方法选择132例全身骨显像表现为单发异常放射性核素分布的患者,局部行SPECT/CT断层扫描。由核医学科2名医生对全身骨显像与SPECT/CT融合显像分别做出诊断,并与随访或病理结果对照,比较两种检查结果的差异性。结果 132例中,经随访或病理最终诊断骨转移瘤73例,骨良性病灶59例。73例骨转移瘤中,全身骨显像检出59例,SPECT/CT融合显像检出71例。SPECT/CT融合显像探测单发骨转移瘤的敏感性、特异性及准确性分别为97.3%、94.9%、96.2%,均明显高于全身骨显像的敏感性、特异性及准确性(80.8%、76.3%、78.8%)(P <0.05)。结论与全身骨显像比较,SPECT/CT融合显像可以明显提高对单发骨转移瘤的诊断效能。 Objective To compare the diagnostic efficacy of whole body bone imaging and SPECT/CT fusion imaging in single bone metastases. Methods A total of 132 patients with single abnormal radionuclide distribution on whole body bone imaging were selected and SPECT/CT tomography was performed locally. Two doctors from the Department of Nuclear Medicine made the diagnosis of the whole body bone imaging and SPECT/CT fusion imaging respectively, and compared the results with follow-up or pathological results, as well as the difference between the two examination results. Results Among the 132 cases, 73 cases were diagnosed as bone metastases and 59 ones as benign bone lesions according to the follow-up or pathology. Among the 73 ones of bone metastases,59 ones were detected by whole body bone imaging and 71 ones by SPECT/CT fusion imaging. The sensitivity, specificity and accuracy of SPECT/CT fusion imaging in detecting single bone metastases were 97.3%, 94.9% and 96.2% respectively, significantly higher than those of whole body bone imaging(80.8%, 76.3%, 78.8%)(P < 0.05). Conclusion Compared with whole body bone imaging, SPECT/CT fusion imaging can significantly improve the diagnostic efficacy of single bone metastases.
