血清孕酮、胰岛素样生长因子在预测妊娠早期不良结局的研究

Serum Progesterone and Insulin-like Growth Factor in Predicting Adverse Outcomes in Early Pregnancy

目的:研究血清孕酮(P)、胰岛素样生长因子等指标在孕早期对妊娠不良结局的预测价值。方法:回顾性分析本院97例先兆流产患者(研究A组)、85例稽留流产患者(研究B组)、正常妊娠孕妇97例(对照组)的资料,均检测P、胰岛素样生长因子1(IGF-1)、胰岛素样生长因子2(IGF-2)、游离β-hCG(fβ-hCG)含量等指标,分析不良妊娠的影响因素。结果:研究A、B组fβ-hCG水平均高于对照组,P、IGF-1、IGF-2水平均低于对照组,差异均有统计学意义(P<0.05);研究B组fβ-hCG水平高于研究A组,P、IGF-1、IGF-2水平低于研究A组,差异均有统计学意义(P<0.05);fβ-hCG病理性增高、P水平降低、IGF-1和IGF-2低表达均是不良妊娠的危险因素(P<0.05)。结论:流产患者P、IGF-1和IGF-2水平不升或降低而fβ-hCG含量居高不下,预示妊娠结局不良。

Objective:To study the predictive value of serum progesterone(P)and insulin-like growth factoron adverse pregnancy outcomes in early pregnancy.Method:The data of 97 threatened abortion patients(study A group),85 missed abortion patients(study B group)and 97 normal pregnant women(control group)in our hospital were retrospectively analyzed.The P,insulin-like growth factor-1(IGF-1),insulin-like growth factor-2 (IGF-2)and free β-hCG(fβ-hCG)were detected,and the influencing factors of adverse pregnancy were analyzed.Result:The levels of fβ-hCG in study A and B groups were higher than those of control group,and the levels of P,IGF-1 and IGF-2 were lower than those of control group,the differences were statistically significant (P<0.05).The level of fβ-hCG in study B group was higher than that of study A group,and the levels of P, IGF-1 and IGF-2 were lower than those of study A group,the differences were statistically significant(P<0.05). Pathological increase of fβ-hCG,decrease of P level and low expression of IGF-1 and IGF-2 were risk factors for adverse pregnancy(P<0.05).Conclusion:The levels of P,IGF-1 and IGF-2 in patients with abortion did not increase or decrease,but the levels of fβ-hCG remained high,indicating poor pregnancy outcome.
