妊娠期高血压疾病孕妇产前心理抑郁状态调查及相关因素分析

Investigation and Analysis of Prenatal Psychological Depression in Pregnant Women with Hypertensive Disorder Complicating Pregnancy

目的:调查妊娠高血压疾病孕妇产前心理抑郁状态,探讨分析其相关影响因素。方法:回顾性分析本院产科110例妊娠期高血压疾病患者相关临床资料,采用抑郁自评量表调查其心理健康状态,根据抑郁自评量表得分将妊娠期高血压疾病孕妇分为抑郁组和非抑郁组,探讨分析妊娠期高血压疾病孕妇发生抑郁的相关因素。结果:在本调查中,妊娠期高血压疾病孕妇产前抑郁的发生率为29.1%,明显高于国内外报道的孕产妇抑郁发生率(P<0.05)。与非抑郁组比较,抑郁组妊娠期高血压疾病孕妇的抑郁自评量表的评分均明显升高(P<0.05)。高龄、单独二胎政策、文化程度低、体力劳动者、家庭环境不良等情况下的妊娠期高血压疾病孕妇产前抑郁发生率更高(P<0.05),并且随着年龄增长、住院时间延长以及并发症增多,抑郁发生率更高,差异均有统计学意义(P<0.05)。结论:妊娠期高血压疾病孕妇心理抑郁的发生率高于一般孕妇群体,且抑郁的发生与年龄、文化程度、职业、家庭状态、住院时间以及合并症等密切相关,应当引起高度重视并尽早干预。

Objective:To investigate the prenatal psychological depression of pregnant women with hypertensive disorder complicating pregnancy,and discussing the related influencing factors.Method:Clinical data of 110 pregnant women with hypertensive disorder complicating pregnancy admitted to the department of obstetrics in our hospital were retrospectively analyzed.The women were divided into depression group and nondepression group according to self ration depression scale(SDS).The related influencing factors were explored and analyzed.Result:In this investigation,the incidence of prenatal psychological depression in pregnant women with hypertensive disorder complicating pregnancy was 29.1%,which was significantly higher than that of the reported incidence of the general pregnant and lying-in woman at home and abroad(P<0.05).The SDS score in depression group was significantly higher than that in the non-depression group(P<0.05).Pregnant women with hypertensive disorder complicating pregnancy had a higher incidence of prenatal depression in the case of old age,single second child policy,low educational level,manual labor,and poor family environment,and the incidence of depression was higher with the increase of age,longer hospitalization time and increased complications,the difference was statistically significant(P<0.05).Conclusion:The incidence of psychological depression in pregnant women with hypertensive disorder complicating pregnancy is higher than that of general pregnant women,and the occurrence of depression is closely related to age,education level,occupation,family status,length of hospitalization and complications,which should be highly valued and intervened as early as possible.
