摘要
目的研究超声对未触及睾丸小儿隐睾的诊断价值。方法选取2012年1月至2017年6月该院收治的未触及睾丸隐睾患儿125例(131侧)作为研究对象,患儿年龄为1岁至11岁零9个月(中位年龄2岁零7个月),术前经彩色多普勒超声确定患侧睾丸的情况,并与手术结果对比分析。结果 131侧未触及睾丸经手术探查证实,腹股沟区隐睾24侧,术前彩色多普勒超声准确检出23侧(敏感度为95.8%);腹腔内睾丸16侧,术前彩色多普勒超声检出其中8侧(敏感度为50.0%);睾丸萎缩残余21侧,术前彩色多普勒超声只发现其中9侧(敏感度为42.9%)。90侧彩色多普勒超声阴性的患者中,82侧(91.1%)术中证实睾丸萎缩残余(12侧)或缺如(70侧),超声对睾丸萎缩残余或睾丸缺如的诊断一致性并不高。结论彩色多普勒超声对未触及睾丸有一定的定位性诊断价值,尤其是腹股沟区未触及隐睾,可避免不必要的腹腔镜探查。
Objective To investigate the diagnostic value of ultrasound in occult cryptorchidism in children without testicular test. Methods A total of 125 children (131 sides) who had not been exposed to testicular cryptorchidism admitted to the hospital from January 2012 to June 2017 were selected as subjects.The age of the children was 1 to 11 years and 9 months (median age was 2 years and 7 months).The condition of the testis was determined by color Doppler ultrasound before operation and compared with the surgical results. Results The sputum 131 side did not touch the testis and was confirmed by surgical exploration.The cryptorchidism in the inguinal region was 24 sides.Preoperative color Doppler ultrasound accurately detected 23 sides (sensitivity was 95.8%).On the 16th side of the testis in the abdominal cavity,8 of them were detected by preoperative color Doppler ultrasound (sensitivity was 50.0%).There were 21 residual testicular atrophy,and only 9 of them were found by preoperative color Doppler ultrasound (sensitivity was 42.9%).Of the 90-negative color Doppler ultrasound-negative patients,82 (91.1%) confirmed intraoperative testicular atrophy (12 sides) or absent (70 side).Ultrasound is not consistent in the diagnosis of testicular atrophy or testicular absence. Conclusion Color Doppler ultrasonography has a certain diagnostic value for untouched testes,especially in the groin area without touching the cryptorchidism,which can avoid unnecessary laparoscopic exploration.
