探讨术前注射VEGF抗体对微创玻璃体手术治疗糖尿病性视网膜病变的影响

The Investigation of the Effect of Preoperative Injection of VEGF Antibody on Minimally Invasive Vitrectomy for Diabetic Retinopathy

目的探讨行微创玻璃体手术前注射抗血管内皮生长因子(VEGF)药物对糖尿病性视网膜病变疗效的影响。方法随机筛选2016年1月～2017年12月期间在本院接受治疗的200例糖尿病性视网膜病变患者,按照双盲法将其分为观察组(n=100)和对照组(n=100)。两组患者均行微创玻璃体手术治疗,观察组患者行玻璃体切除术前3～5d,于玻璃体腔穿刺并注入1mL雷珠单抗,对照组不采取任何操作。评价两组患者的治疗效果,并就两组患者手术前后视力、眼压变化情况进行分析。结果观察组治疗总有效率高于对照组;治疗后,观察组眼压及视力改善情况显著优于对照组,差异有统计学意义(P <0.05)。结论行微创玻璃体手术治疗糖尿病性视网膜病变前注射抗VEGF药物可显著提高手术治疗效果,改善患者症状,促进预后,值得应用。

Objective To investigate the effect of anti-vascular endothelial growth factor (VEGF) injection before minimally invasive vitreous surgery on the outcome of diabetic retinopathy. Methods 200 patients with diabetic retinopathy who were treated in our hospital from January 2016 to December 2017 were randomly divided into observation group (n=100) and control group (n=100). The patients in the two groups all underwent minimally invasive vitrectomy. The patients in the observation group underwent injection of 1 mL of ranibizumab in vitrectomy 3 to 5 days before the vitrectomy surgery. The control group did not take any action. The therapeutic effects of the two groups of patients were evaluated, and the changes of visual acuity and intraocular pressure before and after surgery were analyzed. Results The total effective rate of the observation group was higher than that of the control group. After treatment, the improvement of intraocular pressure and visual acuity in the observation group were significantly better than those of the control group, and the differences were statistically significant (all P < 0.05). Conclusions Preoperative injection of anti-VEGF drugs on minimally invasive vitrectomy for diabetic retinopathy can significantly improve the surgical treatment, patient symptoms, prognosis, and is worthy of application.
