采用不同剂量黄体酮胶囊治疗无排卵型月经失调效果分析

Analysis of the Effect of Different Doses of Progesterone Capsules on Anovulatory Menstrual Disorders

目的探讨不同剂量国产黄体酮胶囊在无排卵型月经失调治疗中的应用效果。方法选择2016年1月～2018年1月于本院就诊的180例无排卵型月经失调育龄期患者作为研究对象,按照黄体酮治疗剂量不同分为100mg、200mg、300mg三个剂量组,每组60例,对比评价不同剂量组患者停药后阴道流血情况,阴道流血量,B超评估子宫内膜厚度,并评估用药后不良反应,包括恶心呕吐、头痛头晕和腹痛等。结果经治疗10天后,3个剂量组患者撤退性出血率分别为95%,98.3%,100%,各组间阴道流血量评分及阴道流血时间比较差异无统计学意义(P> 0.05);三组患者用药前子宫内膜厚度差异无统计学意义,除300mg组出血后子宫内膜厚度较用药前变薄,差异有统计学意义,其余两组用药前后子宫内膜厚度差异无统计学意义。各组患者药物不良反应发生率比较差异无统计学意义。结论针对无排卵型月经失调的治疗,每天口服100、200、300mg国产黄体酮胶囊均能够取得明显的治疗效果,可以采用低剂量药物治疗方案进行治疗。

Objective To investigate the effect of different doses of domestic progesterone capsules on anovulatory menstrual disorders. Methods 180 patients of reproductive age with anovulatory menstrual disorders who were admitted to our hospital from January 2016 to January 2018 were selected as subjects. The doses of progesterone were divided into three groups of 100 mg, 200 mg and 300 mg, with 60 patients each group. After cessation of drug use, vaginal bleeding, B-ultrasound assessment of endometrial thickness were compared in different dose groups, and adverse reactions after medication, including nausea and vomiting, headache, dizziness and abdominal pain were evaluated. Results After 10 days of treatment, the withdrawal bleeding rates of the three dose groups were 95%, 98.3%, and 100%, respectively. There was no significant difference in vaginal bleeding score and vaginal bleeding time length between the groups (P > 0.05). There was no significant difference in the thickness of endometrium before treatment. The thickness of endometrium in the 300mg group was thinner than that before administration (P > 0.05). And no significant difference was observed in the thickness of endometrium between the two groups before and after treatment. No significant difference was observed in the incidence of adverse drug reactions among the groups. Conclusion For the treatment of anovulatory menstrual disorders, daily oral 100, 200, 300 mg domestic progesterone capsules all can achieve significant therapeutic effects, and low-dose drug treatment can be selected.
