农村高血压、糖尿病患者签约服务试点及效果分析

Pilot Family Doctor Contract Service and Effect Analysis of Rural Hypertension and Diabetes Patients

农村高血压、糖尿病患者签约服务试点地区的经验表明,签约服务明显提高了签约患者获得随访服务的密度和质量,签约患者的健康行为开始发生积极的变化、总体健康状况有所改善。借鉴试点地区经验,可完善签约服务包设计、寻求基本医保支持、做好签约医生能力建设和考核评价工作,从而不断推进农村高血压、糖尿病患者签约服务。

The experience of pilot areas in rural high blood pressure and diabetes contracting services showed that the contracting service has significantly improved the density and quality of contracted patients' follow-up services. The healthy behavior of contracted patients began to change positively and the overall health status improved. Drawing on the experience of the pilot areas,we can improve the design of contract service packages,seek basic medical insurance support,and do a good job in contracting doctors' capacity building and assessment and evaluation,so as to continuously promote the signing services for high blood pressure and diabetes patients in rural areas.
