晚睡前口服格列美脲治疗空腹血糖升高为主的2型糖尿病患者的疗效

Efficacy of oral glimepiride administered every night before bedtime for type 2 diabetes mellitus patients with major symptom of elevated fasting blood glucose

目的观察晚睡前口服格列美脲治疗空腹血糖升高为主的2型糖尿病(T2DM)患者的疗效。方法将120例空腹血糖升高为主的T2DM患者,随机分为观察组、对照组A和对照组B,每组40例。3组均给予阿卡波糖和(或)二甲双胍治疗,观察组晚睡前口服格列美脲2～6 mg,对照组A晚餐前口服格列美脲2～6 mg,对照组B晚睡前皮下注射诺和灵N 6～18 U。比较3组患者治疗前后空腹血糖、糖化血红蛋白(HbA1c)水平及治疗期间低血糖发生率。结果治疗后3组患者空腹血糖、HbA1c水平均低于治疗前(P<0.05),并且观察组HbA1c、空腹血糖水平低于对照组A (P<0.05),而观察组与对照组B差异无统计学意义(P>0.05)。观察组低血糖发生率低于对照组B(P<0.05)。结论对于空腹血糖升高为主的T2DM患者,晚睡前口服格列美脲降低空腹血糖的效果好,且低血糖发生率低,可部分替代中效胰岛素。

Objective To observe the efficacy of oral glimepiride administered every night before bedtime for type 2 diabetes mellitus(T2DM) patients with major symptom of elevated FBG. Methods Totally 120 T2DM patients with major symptom of elevated FBG were randomly divided into observation group,control group A and control group B, with 40 cases in each group.All of the three groups were treated with acarbose and/or metformin,besides,the observation group was given oral glimepiride(2-6 mg) every night before bedtime,the control group A underwent oral administration of glimepiride(2-6 mg) right before dinner,the control group B received subcutaneous injection of Novolin N 6-18 U every night before bedtime.Pre- and post-treatment FBG and hemoglobin A1c(HbA1c) levels, along with incidence rate of hypoglycemia during treatment,were compared among the three groups. Results Post-treatment FBG and HbA1c levels were all lower in the three groups than the levels before treatment( P <0.05),moreover,the levels of FBG and HbA1c in the observation group were lower than those in the control group A( P <0.05),but did not statistically differ from those in the control group B( P >0.05).The incidence rate of hypoglycemia in the observation group was lower than that in the control group B( P <0.05). Conclusion For T2DM patients with major symptom of elevated FBG,oral glimepiride administered every night before bedtime obtains favorable efficacy for reducing FBG and a low incidence rate of hypoglycemia,which can partially substitute for intermediate-acting insulin.
