基于护理程序的强化护理干预对学龄期肾病综合征患儿治疗依从性及心理健康行为的影响 被引量:44

Effect of intensive nursing intervention based on nursing procedure on treatment compliance and mental health behavior of school-age children with nephrotic syndrome

摘要 目的探讨基于护理程序的强化护理干预对学龄期肾病综合征(NS)患儿治疗依从性及心理健康行为的影响。方法选择2015年1月至2017年7月新乡医学院第一附属医院收治的90例学龄期NS患儿为研究对象,根据护理措施分为观察组和对照组,每组45例。对照组患儿给予常规护理措施,观察组患儿在常规护理基础上实施基于护理程序的强化护理干预措施。于出院后1 a评估2组患儿治疗依从性,分别于入院时及出院后1 a应用抑郁自评量表(SDS)、焦虑自评量表(SAS)评估患儿抑郁、焦虑情绪,出院后1 a采用自制护理满意度调查问卷评估2组患儿家属满意度。结果观察组和对照组患儿治疗依从率分别为97.78%(44/45)、80.00%(36/45),观察组患儿治疗依从率高于对照组(χ~2=7.205,P<0.05);护理干预前2组患儿SDS、SAS评分比较差异无统计学意义(t=0.386、0.684,P>0.05); 2组患儿护理干预后SDS、SAS评分低于干预前(t=6.652、6.233、11.522、12.787,P<0.05);护理干预后,观察组患儿SDS、SAS评分低于对照组(t=5.515、6.265,P<0.05);观察组和对照组患儿家属满意度分别为93.33%(42/45)、75.56%(34/45),观察组患儿家属满意度高于对照组(χ~2=5.414,P<0.05)。结论基于护理程序的强化护理干预可以缓解学龄期NS患儿焦虑、抑郁情绪,提高治疗依从性和家属满意度,促进患儿康复。 Objective To investigate the effect of intensive nursing intervention based on nursing procedure on treatment compliance and mental health behavior of school-age children with nephrotic syndrome(NS). Methods Ninty school-age children with NS admitted to the First Affiliated Hospital of Xinxiang Medical University from January2015 to July2017 were selected as the research objects,and they were divided into observation group and control group according to the nursing measures,with45 cases in each group.The children in the control group were performed with routine nursing measures, while the children in the observation group were performed with intensive nursing intervention based on nursing procedure on the basis of routine nursing.The treatment compliance of children in the two groups was evaluated at one year after discharge.The depression and anxiety emotions of children were evaluated by self-rating depression scale (SDS) and self-rating anxiety scale (SAS) at admission and one year after discharge.The satisfaction of the children′s families in the two groups was evaluated by self-made nursing satisfaction evaluation questionnaire at one year after discharge. Results The treatment compliance rate of children in the observation group and the control group was 97.78%(44/45) and 80.00%(36/45),respectively.The treatment compliance rate of children in the observation group was higher than that in the control group (χ^2=7.205, P <0.05).There was no significant difference in SDS and SAS scores of children between the two groups before nursing intervention ( t =0.386,0.684;P >0.05).The scores of SDS and SAS after nursing intervention were lower than those before intervention in the two groups ( t =6.652,6.233,11.522, 12.787;P <0.05).The scores of SDS and SAS in the observation group were lower than those in the control group after nursing intervention ( t =5.515,6.265;P <0.05).The satisfaction of the children′s families in the observation group and the control group was 93.33%(42/45) and 75.56%(34/45),respectively.The satisfaction of the children′s families in the observation group was higher than that in the control group (χ^2=5.414, P <0.05). Conclusion Intensive nursing intervention based on nursing procedure can alleviate anxiety and depression of school-age children with NS,improve treatment compliance and family satisfaction,and promote rehabilitation of children.
