Pharmaceutical Care for an Elderly Patient with Non-Hodgkin's Lymphoma Associated with Pulmonary Embolism

目的通过临床药师对一例老年非霍奇金氏淋巴瘤伴肺栓塞患者的药学监护,探讨临床药师如何参与肿瘤患者血栓的预防和治疗。方法对一例肿瘤伴肺栓塞的患者实施全程药学监护,包括协助医师为患者制定抗凝与化疗方案,分析患者国际化标准比值异常波动的可能原因并调整华法林剂量,对患者长期抗凝治疗提供用药教育。结果在临床药师的配合下,医师为患者制定了个体化抗凝和抗肿瘤方案,保证其治疗的有效性和安全性。结论临床药师积极参与肿瘤患者的血栓管理,有利于患者的治疗与预后。

Objective To explore how clinical pharmacists participate in the prevention and treatment of thrombosis for cancer patients by providing full term pharmaceutical care for a patient with non-Hodgkin’s lymphoma associated with pulmonary embolism. Methods Clinical pharmacists participated in the treatment, including the development of anticoagulation therapy and chemotherapy, analysis of possible reasons for the abnormal fluctuation of INR and provision of medical education for the long-term anticoagulation treatment. Results With the help of clinical pharmacists, doctors made individualized anticoagulation and chemotherapy plan to ensure the effectiveness and safety of treatment. Conclusion Clinical pharmacists’ participation in the management of thrombosis of cancer patients is beneficial to the treatment and prognosis of patients.
