摘要
高敏心肌肌钙蛋白(hs-cTn)检测灵敏度高、特异性强,目前已广泛应用于心血管疾病的病情监测及预后评估。随着经济的发展和人们生活方式的改变,心力衰竭的患病率不断增加。心力衰竭患者中hs-cTn升高的相关机制包括心肌细胞直接损伤、心肌细胞氧气供应与需求失衡、内皮细胞功能障碍、心肌细胞凋亡、肾功能不全等。hs-cTn不仅在急性冠状动脉综合征的早期诊断、危险分级和预后方面有重要作用,而且与心力衰竭的严重程度和预后密切相关。进一步了解hs-cTn在心力衰竭中的临床价值,可为今后心力衰竭患者的诊治提供新思路。
The detection of high-sensitivity cardiac troponin(hs- cTn) has high sensitivity and specificity and has been widely used in cardiovascular disease monitoring and prognosis assessment.With the development of economy and the change of people′s life style, the prevalence of heart failure is increasing.The mechanisms of elevated hs-cTn in patients with heart failure include direct injury of cardiomyocytes,imbalance of oxygen supply and demand of cardiomyocytes, dysfunction of endothelial cells,cardiomyocyte apoptosis,renal insufficiency and so on. Recent studies have found that hs-cTn not only plays an important role in the early diagnosis, risk classification and prognosis of acute coronary syndrome,but is also closely related to the severity and prognosis of heart failure.Therefore,further understanding of the clinical value of hs-cTn in heart failure can provide a new idea for the diagnosis and treatment of heart failure in the future.
