冠心病心力衰竭患者冠脉病变程度与心功能、心肌酶谱的关系分析 被引量:8

Analysis of the relationship between the degree of coronary artery lesion and cardiac function and myocardial enzyme spectrum in patients with coronary heart disease and heart failure

摘要 目的探讨冠心病心力衰竭患者冠脉病变程度与心功能、心肌酶谱的关系。方法参照冠脉Gensini评分将我院收治的180例冠心病心力衰竭患者分为冠脉轻度病变组(A组)、冠脉中度病变组(B组)与冠脉重度病变组(C组),各60例;另选取60例体检健康志愿者为对照组。比较四组心脏超声检查结果及心肌酶谱检测结果。结果四组LVDD、LVDA、EF、SV、A/E比较,差异显著(P<0.05);LVDD、LVDA、A/E随冠脉病变程度的加重而升高,EF、SV随冠脉病变程度的加重而降低(P<0.05)。CK、CK-MB、LDH、α-HBDH、AST水平均随冠脉病变程度的加重而升高(P<0.05)。结论冠心病心力衰竭患者的冠脉病变程度越严重,其心功能越差,心肌酶谱水平越高。 Objective To investigate the relationship between the degree of coronary artery lesion and cardiac function and myocardial enzyme spectrum in patients with coronary heart disease and heart failure.Methods A total of 180 patients with coronary heart disease and heart failure in our hospital were divided into mild coronary artery lesion group (group A),moderate coronary artery lesion group (group B) and severe coronary artery lesion group (group C) according to Gensini scores,with 60 cases in each group.Another 60 healthy volunteers were selected as control group.The results of cardiac ultrasonic examination and myocardial enzyme spectrum examination were compared among the four groups.Results There were significant differences in LVDD,LVDA,EF,SV and A/E among the four groups (P<0.05);the LVDD,LVDA and A/E increased with the severity of coronary artery lesion,while the EF and SV decreased with the severity of coronary artery lesion (P<0.05).The levels of CK,CK-MB,LDH,α-HBDH and AST increased with the severity of coronary artery lesion (P<0.05).Conclusion The severer the severity of coronary artery lesion,the worse the cardiac function and the higher the myocardial enzyme spectrum levels in patients with coronary heart disease and heart failure.
