布地奈德治疗小儿哮喘急性发作的效果及其对炎症介质水平和肺功能的影响

Effect of budesonide in the treatment of acute attack of asthma in children and its effect on levels of inflammatory mediators levels and pulmonary function

目的探析布地奈德用于小儿哮喘急性发作治疗中的效果及对患儿炎症介质水平及肺功能的影响。方法选取2017年1月至2018年8月渭南市第一医院收治的哮喘急性发作的患儿80例为研究对象,采用随机数字表法将患儿平均分为对照组和研究组,各40例。对照组患儿采用孟鲁司特钠治疗,研究组患儿在对照组的基础上加用布地奈德雾化吸入治疗。比较两组患儿的治疗效果。结果治疗后,两组患儿日间症状评分和夜间症状评分均明显降低,且研究组低于对照组(P<0.05);两组患儿TIMP-1水平均明显升高,MMP-9水平和MMP-9/TIMP-1均明显降低,且研究组优于对照组,差异具有统计学意义(P<0.05);两组患儿的HMGB1、Periostin、Eotaxin和ADAM33水平均明显降低,且研究组低于对照组(P<0.05);两组患儿FVC、FEV1及FEV1/FVC均明显升高,且研究组高于对照组(P<0.05)。结论布地奈德治疗小儿哮喘急性发作效果显著,不仅可提高局部抗炎作用,缓解患儿的症状,还可改善患儿的肺功能,值得推广。

Objective To explore the effect of budesonide in the treatment of acute attack of asthma in children and its effect on levels inflammatory mediators and pulmonary function.Methods Eighty children with acute asthma attack admitted to the First Hospital in Weinan from January 2017 to August 2018 were selected as the research objects.The children were divided into control group and study group by random number table method,with 40 cases in each group.The control group was treated with montelukast sodium,while the study group was treated with budesonide on the basis of the control group.The therapeutic effect were compared between the two groups.Results After treatment,the scores of day and night symptoms in the two groups decreased,and those in the study group were significantly lower than the control group (P<0.05);the TIMP-1 levels in both groups significantly increased,MMP-9 levels and MMP-9/TIMP-1 significantly decreased,and those in the study group were better than the control group,the differences were statistically significant(P<0.05);the levels of HMGB1,Periostin,Eotaxin and ADAM33 in the two groups significantly decreased,and those in the study group were lower than the control group (P<0.05);the levels of FVC,FEV1 and FEV1/FVC in the two groups increased,and those in the study group were significantly higher than the control group (P<0.05).Conclusion Budesonide is effective in the treatment of acute attack of asthma in children.It can not only improve the local anti-inflammatory effect,relieve the symptoms of children,but also improve the pulmonary function of children,which is worthy of promotion.
