显微手术与常规开颅切除手术治疗脑胶质瘤患者的对比研究

Comparative Study of Microsurgery and Conventional Craniotomy for Glioma Patients

目的:对比分析显微手术与常规开颅切除手术治疗脑胶质瘤患者的临床效果。方法:选取2014年8月～2016年7月我院68例脑胶质瘤患者,依据治疗方案不同分为显微组与开颅组,各34例。开颅组行常规开颅切除手术,显微组行显微手术。统计对比两组手术相关指标(术中出血量、手术、术后下床及住院时间)治疗效果、术后并发症发生率,随访2年,对比两组术后1年、2年复发情况。结果:显微组术中出血量较开颅组少,手术、术后下床及住院时间均较开颅组短(P<0.05);显微组治疗总有效率97.06%较开颅组76.47%高(P<0.05);显微组术后并发症发生率5.88%较开颅组29.41%低(P<0.05);两组随访1年复发率相比,无明显差异(P>0.05);随访2年,显微组复发率8.82%较开颅组29.41%低(P<0.05)。结论:显微手术治疗脑胶质瘤可明显减少术中出血量,缩短手术用时,进一步提高治疗效果,明显降低术后并发症发生率,促进患者术后早期恢复,且复发率较低,值得临床推广应用。

Objective: To Compare the clinical effects of microsurgery and conventional craniotomy in the treatment of glioma patients. Methods: 68 patients in our hospital from August2014 to July2016 were divided into2 groups34 cases in each one. The results of the two groups of surgical related indexes and postoperative complications were compared. The follow-up period was 2 years. The recurrence of1year and2 years after operation was compared. Results: The amount of bleeding in the microsurgery group was less than the craniotomy group. The time of surgery, postoperative bed and hospital stay were shorter than the craniotomy group (P<0.05). The total effective rate of the microscopic group was 97.06% higher than the craniotomy group.(P<0.05);the incidence of postoperative complications was 5.88% in the microsurgery compared with29.41% in the craniotomy group (P<0.05). There was no significant difference in the recurrence rate between the two groups (P>0.05). After2 years, the recurrence rate of the microscope was 8.82% lower than that of the craniotomy group (29.41%)(P<0.05). Conclusion: Microsurgical treatment of glioma can significantly reduce the amount of intraoperative blood loss, shorten the time of surgery, further improve the treatment effect, significantly reduce the incidence of postoperative complications, promote early recovery, and the recurrence rate is low.
