纵切横缝内括约肌切断术对老年肛裂患者临床治疗效果及安全性研究

Clinical efficacy and safety of longitudinal sphincterotomy in elderly patients with anal fissure

目的:探讨分析老年肛裂临床治疗方式选择纵切横缝内括约肌切断术的临床疗效以及安全性。方法:对30例老年肛裂患者展开研究分析,按照随机方式分组,对照组给予开放式侧位肛裂切除术,观察组给予纵切横缝内括约肌切断术,对比其疗效。结果:观察组15例患者中治愈、好转以及无效的例数分别为9例、5例、1例,治疗总有效率为93.3%,对照组15例患者中治愈、好转以及无效的例数分别为6例、6例、3例,治疗总有效率为80.0%,组间差异有统计学意义(P<0.05);观察组患者切口愈合时间、肛门畸形率以及并发症发生率显著优于对照组,差异有统计学意义(P<0.05)。结论:老年肛裂治疗方式选择纵切横缝内括约肌切断术疗效显著,可减轻患者的肛门疼痛,使患者正常排便,缩短切口的愈合时间,减少肛门畸形情况的发生,更加安全有效。

Objective To investigate the clinical efficacy and safety of longitudinal and transverse internal sphincterotomy in the treatment of senile anal fissure. Method 30 elderly patients with anal fissure received in our hospital were studied and analyzed.They were randomly divided into two groups.The control group was treated with open lateral anal fissure resection,and the observation group was treated with longitudinal and transverse internal sphincterotomy.The curative effect of two groups was compared.Results There were 9 cases,5 cases and 1 case in the observation group.The total effective rate was 93.3%.In the control group,6 cases,6 cases and 3 cases were cured,improved and ineffective.The total effective rate was 80.0%.There was significant difference between the two groups (P<0.05).The incision healing time,the rate of anorectal malformation and the incidence of complications in group A were significantly better than those in group B,with significant difference (P<0.05).Conclusion The effect of longitudinal and transverse internal sphincterot omy is remarkable.It can alleviate anal pain,make patients defecate normally,shorten the healing time of incision,reduce the occurrence of anomaly,and is more safe and effective.
