摘要 目的:开发重症监护室(ICU)压疮风险管理体系,提高护理管理部门对压疮防治质量控制的工作效率。方法:将重症监护系统与医院信息系统信息共享,在重症监护系统中植入风险管理,设计预防观察、评价和处理3个模块,开发ICU压疮风险管理体系,指导并全面记录护理工作,以及压疮事件的网络追踪。结果:ICU压疮风险管理体系的应用,实现压疮信息和知识共享,减少护士压疮记录和上报的不准确性,压疮信息记录交接准确性由纸质交接单的78%提升至97%,压疮不良事件动态追踪率达100%。结论:ICU压疮风险管理体系使信息涵盖内容真实准确、客观全面,可对患者实施压疮风险前瞻性监控,落实干预措施,全面掌控压疮不良事件,保障护理安全。 Objective: To develop a risk management system of pressure ulcer in intensive care unit (ICU) so as to improve the work efficiency of nursing management department in the quality control of prevention and treatment for pressure ulcer. Methods: Through the information sharing of intensive care system and the hospital information system and implanting the risk management in the intensive care system, the risk management system of pressure ulcer in ICU was developed by designing three modules included prevention observation, evaluation and treatment to guide and comprehensively record the nursing work, as well as the network tracking of pressure ulcer events. Results: The application of risk management system of pressure ulcer in ICU realized the sharing of information and knowledge about pressure ulcer, and reduce the inaccuracy of record and report of pressure ulcer information of nurses. And the switching accuracy of information record about pressure ulcer was increased from 78% of paper switch sheet to 97%, and the dynamic tracking rate of adverse events of pressure ulcer achieved to 100%. Conclusion: The risk management system of pressure ulcer in ICU enables the contained content of information to be true, accurate, objective and comprehensive, and it can carry out prospective monitoring for pressure ulcer risk of patients, and implement intervention measures, and comprehensively control adverse events of pressure ulcer and ensure the safety of nursing.
