不同生物材料在年轻恒前牙外伤直接盖髓术后的应用效果 被引量:6

Application effect of different biomaterials after direct pulp capping of young permanent anterior teeth trauma

摘要 目的:观察不同生物材料在年轻恒前牙外伤直接盖髓术后的应用效果。方法:选取我院于2011年3月至2016年8月收治的50例恒前牙外伤患儿为研究对象,根据入组时间编号并随机分为A、B组,25例(30颗)/组,两组行直接盖髓术时A组以无机三氧化物聚合体(MTA)为盖髓材料,而B组则以Theracal为盖髓材料。比较两组采用不同生物材料盖髓后的临床疗效,以及牙本质增加量、牙根吸收程度和不良反应发生情况。结果:A组患儿治疗成功率(93. 33%)显著高于B组(73. 33%),具有统计学差异(P <0. 05)。A组在0~6个月以及0~12个月内的牙本质增加量(0. 124±0. 013 mm,0. 179±0. 014 mm)显著高于B组患儿(0. 091±0. 011 mm,0. 151±0. 019 mm),具有统计学差异(P <0. 05),在6~12个月内的牙本质增加量与B组患儿无统计学差异(P> 0. 05)。A组患儿0~6个月、6~12个月牙根吸收程度显著优于B组,具有统计学差异(P <0. 05)。两组治疗期间均未发生明显的不良反应。结论:MTA在年轻恒前牙外伤直接盖髓术后的应用效果显著优于Theracal,安全性较高,有利于预后,值得临床推广。 Objective: application effect of different biomaterials after direct pulp capping of young permanent anterior teeth trauma. Methods: 50 cases of permanent anterior tooth trauma were selected in our hospital from March 2011 to August 2016 and randomly divided into A and B groups according to the enrollment time number,25 cases( 30 teeth) per group. The two groups were treated with direct pulp capping using mineral trioxide aggregate group A used mineral trioxide aggregate and group B Theracal. The clinical effects,dentin increment,dentin thickness,root resorption and adverse reactions were compared between the two groups. Results: The success rate of group A( 93. 33%) was significantly higher than that of group B( 73. 33%),with a statistical significance( P <0. 05). The dentin increment of group A( 0. 124 ± 0. 013 mm,0. 179 ± 0. 01 mm) was significantly higher than that in group B( 0. 091 ± 0. 011 mm,0. 151 ± 0. 019 mm) in 0 ~ 6 months and 0 ~ 12 months,and the difference was significant( P < 0. 05). There was no significant difference in the increase of dentin between the two groups within 6 ~ 12 months( P > 0. 05). The dentin thickness of group A was significantly higher than that of group B in0 ~ 6 months and 6 ~ 12 months,with statistical significances( P < 0. 05). The root resorption of group A was significantly better than that of group B in 0 ~ 6 months and 6 ~ 12 months,with statistical significance( P < 0. 05).There were no obvious adverse reactions in the two groups during treatment. Conclusion: The application of MTA after direct pulp capping of young permanent anterior teeth is superior to that of Theracal,which is beneficial to prognosis and is worth popularizing.
