优质护理结合强化肠外营养支持对胃癌合并肠梗阻患者免疫功能及生活质量的影响

Effect of high quality nursing combined with enhanced parenteral nutrition support on immune function and quality of life in patients with gastric cancer complicated by intestinal obstruction

目的研究优质护理结合强化肠外营养支持对胃癌合并肠梗阻患者免疫功能及生活质量的影响。方法胃癌合并肠梗阻患者70例,使用奇偶数法分为常规组和试验组,每组35例。常规组采用常规护理,试验组给予优质护理结合强化肠外营养支持,比较2组干预前后免疫功能及生活质量。结果干预前,2组IgA、IgG、IgM、CD^+_3、CD^+_4、CD^+_4/CD^+_8及生活质量评分差异无统计学意义(P>0.05);干预后2组生活能力、生理职能、社会功能3个维度差异无统计学意义(P>0.05);试验组IgA、IgG、IgM、CD^+_3、CD^+_4、CD^+_4/CD^+_8水平及健康、生理机能、精神健康、情感职能及生活活力5个维度评分显著高于常规组,差异有统计学意义(P<0.05)。结论优质护理结合强化肠外营养支持能有效改善胃癌合并肠梗阻患者免疫功能,提高其生活质量,值得临床推广。

Objective To investigate the effects of high quality nursing combined with enhanced parenteral nutrition support on immune function and quality of life in patients with gastric cancer complicated by intestinal obstruction.Methods A total of 70 patients with gastric cancer complicated by intestinal obstruction who were admitted and treated in our hospital from May 2014 to December 2017 were divided into control group(n=35) and experimental group(n=35) according odd even method. The patients in control group were treated by routine nursing, however,the patients in experimental group were given high quality nursing combined with enhanced parenteral nutrition support. The immune function and quality of life before and after intervention were observed and compared between the two groups.Results Before intervention, there were no significant differences in IgA, IgG, IgM, CD3^+, CD4^+, CD4^+/CD8^+ and quality of life scores between the two groups(P>0.05). After intervention, there were no significantly differences in the three dimensions of life ability, physiological function and social function between the two groups(P>0.05). However the levels of IgA, IgG, IgM, CD3^+, CD4^+, CD4^+/CD8^+ and the five dimensions of health, physiology, mental health, emotional function and life vitality in experimental group were significantly higher than those in control group(P<0.05).Conclusion High quality nursing combined with enhanced parenteral nutrition support can effectively improve the immune function and enhance the quality of life of patients with gastric cancer complicated by intestinal obstruction,therefor, which is worthy of clinical promotion.
