摘要
目的研究联合检测血清半乳糖凝集素-3(galectin-3)、可溶性白细胞介素2受体(soluble interleukin 2 receptor,sIL 2R)、嗜酸性粒细胞趋化因子(CC chemokine ligand 11,eotaxin,CCL11)表达水平在评估结肠癌患者预后中的价值研究。方法选取2012年2月至2013年2月住院的结肠癌手术患者92例为研究对象,另选同期体检正常者92例为对照组,患者均进行临床资料收集,分别对患者的TNM肿瘤分期、肿瘤位置、体积、分化程度、转移情况、浸润情况、病理类型、生存情况进行统计,同时检测2组血清脂肪因子[血清galectin-3、sIL-2R、CCL11]水平,并绘制受试者工作特征(receiver operating characteristic curve,ROC)曲线,评价三者单独及联合检测对结肠癌患者预后价值。结果研究组患者中,不同性别、年龄、肿瘤位置、大小、浸润深度、脉管癌栓情况、TNM分期情况、病理类型患者的血清galectin-3、sIL-2R、CCL11水平之间差异无统计学意义(P>0.05)。淋巴结转移超过12枚、肿瘤复发以及五年随访死亡患者的血清galectin-3、sIL-2R、CCL11水平明显升高,差异有统计学意义(P<0.05);患者血清galectin-3、sIL-2R、CCL11水平越高,患者淋巴结转移情况、肿瘤复发情况以及随访死亡情况越高;血清galectin-3、sIL-2R、CCL11均为结肠癌患者五年生存的独立危险因素;血清galectin-3、sIL-2R、CCL11单项检测诊断急性脑梗死的特异度比较差异有统计学意义(P<0.05),galectin-3+sIL-2R+CCL11诊断的敏感度和特异度显著高于galectin-3+CCL11、sIL-2R+CCL11及galectin-3+sIL-2R(P<0.05)。结论对于早期结直肠癌患者,术前血清galectin-3、sIL-2R、CCL11联合检测对于识别预后不良的患者具有重要的临床意义。
Objective To explore the application value of combination detection of serum levels of galectin-3, soluble interleukin 2 receptor (sIL 2R), and CC chemokine ligand 11 (CCL11) in evaluating the prognosis of patients with colon cancer. Methods A total of 92 patients with colon cancer who were admitted and treated by surgery in our hospital from February 2012 to February 2013 were enrolled as research subjects,at the same time, the other 92 patients who underwent physical examination at the same period were enrolled as control group. The clinical data of patients were collected, and the TNM tumor stage, tumor location, tumor volume, degree of differentiation, metastasis, infiltration, pathological type, and survival were calculated. The levels of serum adipokines [serum galectin-3, sIL-2R, and CCL11] were measured in both groups, and the receiver operating characteristic curve (ROC) was drawn. The application value of simple detection and combination detection of serum levels of galectin-3, sIL-2R, and CCL11 in evaluating the prognosis of patients with colon cancer was assessed. Results There were no significant differences in serum levels of galectin-3, sIL-2R, and CCL11 in patients with different gender, age, tumor location, size, depth of invasion, vascular tumor thrombus, TNM stage, and pathological type in research group ( P >0.05). The serum levels of galectin-3, sIL-2R, and CCL11 were significantly increased in patients with the number of metastasized lymph node≥12, tumor recurrence, and death after 5-year follow-up, and the differences were statistically significant ( P <0.05). The higher the level of serum galectin-3, sIL-2R and CCL11, the higher the lymph node metastasis, tumor recurrence and follow-up death.The serum levels of galectin-3, sIL-2R and CCL11 were independent risk factors for five-year survival of colon cancer patients. There was significant differences in the specificity of serum galectin-3, sIL-2R and CCL11 in the diagnosis of acute cerebral infarction ( P <0.05), and the sensitivity and specificity of galectin-3+ sIL-2R + CCL11 in the diagnosis of colon cancer were significantly higher than those of galectin- 3 + CCL11, sIL-2R + CCL11 and galectin-3+ sIL-2R ( P < 0.05 ). Conclusion For patients with early colorectal cancer,preoperative combination detection of serum levels ofgalectin-3, sIL-2R and CCL11 is of great clinical significance in identifying the patients with poor prognosis.
