HIV-1感染患者肠道归巢CD4^+变化与T淋巴细胞亚型的相关性 被引量:4

摘要 目的探讨Ⅰ型艾滋病病毒(HIV-1)感染患者肠道归巢CD4^+细胞的变化特点与T淋巴细胞亚型的相关性。方法回顾性选取广西艾滋病临床治疗中心艾滋病外科收治的40例HIV-1感染者作为研究对象,按病情分为慢性感染者18例,急性感染者22例,同时选取20例健康人作为对照,分别观察三组受试者肠道归巢CD4^+细胞数量及比率,并分析急性感染患者肠道归巢CD4^+细胞与T淋巴细胞亚型的相关性。结果 HIV-1急性期感染患者肠道归巢CD4细胞比率、CD4细胞数量显著低于健康对照组,差异具有统计学意义(P <0. 05); HIV-1慢性期感染患者肠道归巢CD4细胞比率、CD4细胞数量显著低于HIV-1急性期感染患者,差异具有统计学意义(P <0. 05)。HIV-1急性期感染者的肠道归巢CD4细胞的数量与外周血CD4细胞计数呈正相关(r=0. 603,P <0. 001),与CD4/CD8细胞比率呈正相关(r=0. 517,P=0. 005); HIV-1急性期感染者的肠道归巢CD4细胞的数量与血浆中HIV病毒载量呈负相关(r=-0. 801,P <0. 001),与T细胞活化水平呈负相关(r=-0. 647,P=0. 001)。结论肠道归巢CD4细胞可在HIV-1急性感染后降低,在HIV-1感染中有重要意义。 Objective To investigate the correlation between intestinal homing CD4+cells changes and T lymphocyte subsets in HIV-1 infected patients.Methods 40 patients with HIV-1 infection admitted in Guangxi AIDS Clinical Treatment Center of AIDS surgery were divided into chronically infected with 18 patients and acutely infected with 22 patients according to the condition,another 20 healthy controls also enrolled.The number and proportion of homing CD4^+cells of three groups were observed.The correlation between intestinal homing CD4^+cells and T lymphocyte subtypes in acutely infected patients was analyzed.Results The percentage and number of CD4 cells in whole blood of patients with acute HIV-1 infection were significantly lower than those of healthy control group and higher than those of chronic infection group(P<0.05).The number of CD8 cells in patients with acute HIV-1 infection was higher than that of healthy control group and chronic infection group,and the ratio of CD8 cells was higher than that of control group,lower than that of chronic infection group,with statistical significance(P<0.05).The proportion of intestinal homing CD4 cells,the number of intestinal homing CD4 cells in patients with acute HIV-1 infection were significantly lower than those in healthy controls(P<0.05).The proportion of intestinal homing CD4 cells in HIV-1 chronic infection patients was significantly lower than that in healthy controls(P<0.05).The number of intestinal homing CD4 cells,the number of intestinal homing CD4 cells in patients with acute HIV-1 infection was significantly lower than that in healthy controls(P<0.05).The number of intestinal homing CD4 cells in HIV-1 infected patients was positively correlated with CD4 cell count in peripheral blood(r=0.603,P<0.001),and positively correlated with CD4/CD8 cell ratio(r=0.517,P=0.005).The number of intestinal homing CD4 cells in HIV-1 acute phase infection patients was negatively correlated with HIV viral load in plasma(r=-0.801,P<0.001),and negatively correlated with T cell activation level(r=-0.647,P=0.001).Conclusion Intestinal homing CD4 cells can be reduced after acute HIV-1 infection,which is of great significance in HIV-1 infection.
