乳腺癌临床病理特征对乳腺癌肝转移的影响

Effect of clinicopathologic characteristics of breast cancer on breast cancer liver metastasis

目的分析乳腺癌患者术后病理学特征、治疗以及生存情况,探讨影响乳腺癌肝转移的因素。方法收集1 801例经病理证实的乳腺癌术后患者,回顾性分析了其年龄,临床病理特征,手术方式,放疗与否,以及乳腺癌术后至发生肝转移的时间。通过卡方检验分析患者临床病理特征与乳腺癌肝转移之间的相关性,通过生存分析研究影响乳腺癌肝转移的因素。结果卡方检验显示:腋下淋巴结状态(P=0.000)、是否放疗(P=0.014)与乳腺癌术后发生肝转移具有相关性。单因素分析显示:腋下淋巴结状态(P=0.000)、是否放疗(P=0.012)与乳腺癌术后至肝转移发生的时间之间存在统计学差异。将单因素分析有意义的2个因子纳入Cox回归模型进行多因素预后分析,只有腋下淋巴结状态是乳腺癌肝转移的独立预后因素(P=0.000)。结论腋下淋巴结状态是乳腺癌肝转移的独立预后因素。

Objective To investigate clinicopathologic characteristics,treatment and survival of breast cancer patients,and risk factors on breast cancer liver metastasis.Methods The clinical data of clinicopathologic characteristics,therapies and liver metastasis-free survival in 1 801 breast cancer patients confirmed by pathology were retrospectively analyzed.Chi-square test was used to analyze correlation between clinicopathologic characteristics and breast cancer liver metastasis.Survival analysis was then applied to discuss influencing factors of breast cancer liver metastasis.Results Chi-square test indicated that axillary lymph node status(P=0.000)and radiotherapy(P=0.014)were associated with breast cancer liver metastasis.Univariate analysis showed that axillary lymph node status(P=0.000)and radiotherapy(P=0.012)were significantly associated with liver metastasis-free survival.Variables significantly prognostic in the univariate analysis were selected for the multivariable analysis and only axillary lymph node status(P=0.000)remained significant independent factor of breast cancer liver metastasis.Conclusion Axillary lymph node status is significant independent factor of breast cancer liver metastasis.
