2016-2017年天津市环湖医院病原菌分布特点及药敏实验结果分析

摘要 目的:了解2016-2017年天津市环湖医院分离鉴定的病原菌分布特点及耐药性情况,为临床合理使用抗生素提供参考。方法:回顾性分析了2016-2017年我院临床各科室送检的各种类型标本检出并鉴定的病原菌及药物敏感性试验结果。结果:2016和2017全年分别分离并鉴定出病原菌259[株和2460株,检出阳性率分别为36.4%和37.8%。其中革兰氏阴性杆菌分别为1858株和1815株,占和73.8%,其中肺炎克雷伯菌、鲍曼不动杆菌、铜绿假单胞菌、大肠埃希菌、产气肠杆菌、粘质沙雷菌是主要致病阴性菌.2016年和2017年革兰氏阳性球菌分别为683株和619株,占26.4%>和25.1%,其中金黄色葡萄球菌,表皮葡萄球菌、粪肠球菌、屎肠球菌是主要的致病阳性菌、2016年和2017年真菌分别为50株和27株,占1.9%和1.1%。革兰氏阴性杆菌中鲍曼不动杆菌和大肠埃希氏菌耐药比较普遍,革兰氏阳性球菌中金黄色葡萄球菌、表皮葡萄球菌对青霉素耐药率较高大于80%。结论:革兰氏阴性杆菌为我院2016至2017年细菌感染的主要致病菌,药敏试验结果对临床合理选用抗生素有指导作用,临床应根据药敏结果合理使用抗生素。 Objective: This study aims to investigation the distribution characteristics and drug resistance of pathogenic bacteria derived from clinical specimens in Tianjin huanhu hospital from 2016 to 2017, and to guide the rationally use of antibiotics. Methods: All kinds of clinical specimens from Jannuary 2016 to December 2017 were retrospective an aiyzeds. Results: A total of 2591 and 2460 strains of pathogens were isolated respectively in 2016 and 2017 including 1858 strains (71.7%) and 1815 strains (73.8%) of Gram-negative bacilli, 683 strains (26.4%) and 619 strains (25.1%) of Gram-positive bacilli and 50 strains (1.9%) and 27 (1.1%) of fungi. The dominant Gramnegative strains were Klebsiella pneumonia, acinetobacter baumannii, pseudomonas aeruginosa, Escherichia coli, enterobacter aerogenes, serratia marcescens.The dominant Gram-positive strains were staphylococcus epidermal staphylococcus, enterococcus faecalis, enterococcus faecalis, staphylococcus aureus. Drug sensitivity test showed that the drug resistance of acinetobacter baumannii and escherichia coli was relatively common. There were high drug resistance of staphylococcus aureus and staphylococcus epidermis for penicillin (more than 80%). Conclusion: Gram-negative bacilli is the main pathogen in our hospital, and the results of drug sensitivity test have a guiding effect for the rational selection of antibiotics in clinical practice, and antibiotics should be used rationally according to the drug sensitivity test.
