进行性骨化性纤维发育不良^99Tc^m-MDP SPECT/CT显像一例 被引量:5

^99Tc^m-MDP SPECT/CT in fibrodysplasia ossificans progressiva: a case report

摘要 患者男,22岁。16岁时突发双髋部疼痛性肿块,外院诊断为双髋部软组织肿瘤并行手术切除,术后病理提示骨化性肌炎。术后1个月患者出现双髋部活动受限,难以正常保持坐姿,后逐渐出现双侧髋部屈曲、外展功能障碍,目前已丧失功能。自诉出生时双足拇趾较短小,无家族遗传疾病史。体格检查:双髋部屈曲、外展几乎无活动度,左胸部活动受限,呼吸运动功能受损。实验室检查血红细胞沉降率、血液生化、C反应蛋白等无异常。颈椎正侧位X线片(图1A):颈后软组织内见条形高密度影,骨化性肌炎?双足X线正斜位片(图1B):双足拇趾近远节趾骨融合。采用荷兰Philips公司Precedence SPECT/CT仪行99Tcm-亚甲基二膦酸盐(methylene diphosphonate, MDP;上海原子科兴药业有限公司产品,放化纯>95%)全身骨显像(图2):双肩胛骨内侧、左侧肋骨外侧、腰椎、骨盆左侧、右股骨头外侧见条状放射性分布增高区,双足拇趾短小。胸部SPECT/CT显像示:双肩胛骨内侧、左侧肋骨、胸椎后方肌肉或肌间隙内见多发大小不等条状骨性密度影,左侧自肩胛骨至左骨盆连成片,放射性分布增高。综合患者病史、特征性拇趾短小畸形及影像学特征,临床诊断为进行性骨化性纤维发育不良(fibrodysplasia ossificans progressiva, FOP)。
