丹参酮联合优甲乐对桥本甲状腺炎伴甲状腺功能减退患者免疫功能的影响

Effects of Tanshinone and Euthyrox on Immunologic Function of Hashimoto's Thyroiditis Accompanying Hypothyroidism

目的:观察丹参酮联合优甲乐治疗桥本甲状腺炎(HT_)伴甲状腺功能减退(甲减)的疗效及其对患者免疫功能的影响。方法:将HT合并甲减患者84例根据治疗方法的不同分为观察组和对照组各42例。对照组予优甲乐治疗,观察组在对照组治疗的基础上予丹参酮治疗。观察2组患者疗效及治疗前后游离甲状腺素T_3、T_4(FT_3、FT_4)、促甲状腺激素(TSH)、甲状腺过氧化物酶抗体(TPO-AB)、白细胞介素(IL-10、IL-12、IL-4)和γ干扰素(IFN-γ)、CD3^+、CD4^+、CD8^+和CD4^+/CD8^+水平。结果:总有效率观察组为95.2%,高于对照组的76.2%,2组比较差异有统计学意义(χ~2=4.764,P<0.05)。治疗后2组患者FT_3、FT_4、IL-10、IL-4、CD3^+和CD8^+水平较治疗前升高(P<0.01),TSH、TPO-AB、IFN-γ、IL-12、CD4^+和CD4^+/CD8^+水平降低(P<0.01),观察组变化程度较对照组明显(P<0.01)。结论:丹参酮联合优甲乐能够缓解HT伴甲减患者机体Th1/Th2漂移,纠正机体免疫紊乱。

Objective: To observe clinical effects of tanshinone joined with euthyrox in treating Hashimoto's thyroiditis (HT) accompanying hypothyroidism and its effects on immunologic function. Methods: According to different therapeutic methods,84 patients suffering HT accompanying hypothyroidism were allocated into the observation group and the control group, 42 cases each group. The control group took euthyrox, and the observation group was given tanshinone based on euthyrox. To observe clinical effects of both groups, the levels of FT3, FT4, TSH, TPO-AB, IL-10, IL-12, IL-4, IFN-γ, CD3^+, CD4^+ and CD4^+/CD8^+. Results: Total effective rate of the observation group was 95.2%, superior to 76.2% of the control group, and the difference had statistical meaning (x^2=4.764, P<0.05). After treating, the levels of FT3, FT4, IL-10, IL-4, CD3^+ and CD8^+ rose compared with before treating (P<0.01);the levels of TSH, TPO-AB, IFN-γ, IL-12, CD4+ and CD4^+/CD8^+ lowered (P<0.01), the changes of the observation group were more notable than the control group (P<0.01). Conclusion: Tanshinone joined with euthyrox in treating HT accompanying hypothyroidismcould relieve Th1/Th2 shift and correct immune disorders.
