血浆Septin 9甲基化在结直肠癌治疗中的监测作用

Monitoring Role of Blood Plasma Septin 9 Methylation in the Treatment of Colorectal Carcinoma

为探讨血浆Septin 9甲基化在结直肠癌治疗中的监测作用,选取我院收治的64例结直肠癌患者为观察组,同期健康体检者64名为对照组。收集患者空腹静脉血,采用实时荧光探针PCR法检测Septin 9基因甲基化,比较结直肠癌患者与健康体检者及结直肠癌患者治疗前后的血浆Septin 9甲基化水平。结果显示,观察组Septin 9甲基化阳性表达47例,阴性表达17例,阳性表达率为73.4%。对照组Septin 9甲基化阳性0例,阴性64例,阳性率为0,2组比较差异有统计学意义(P <0.05)。观察组中接受手术治疗49例,化疗15例。手术治疗者中术前Septin 9甲基化阳性37例,阴性12例;术后27例阳性者转阴。化疗者中化疗前Septin 9甲基化阳性10例,阴性5例;化疗后1例阳性者转阴。术后随访1年,结直肠癌术后复发或远处转移者19例。其中Septin 9甲基化阳性14例,阳性率73.7%,血清CEA阳性7例,阳性率36.8%。2者阳性率比较差异有统计学意义(P <0.05)。结果表明,Septin 9甲基化检测能提高结直肠癌诊断准确率,可作为结直肠癌疗效评价及术后随访监测的分子标志物。

This study was to investigate the monitoring role of blood plasma Septin 9methylation in the treatment of colorectal carcinoma,enrolled 64cases in author's hospital as observation group,at the same time,64healthy persons as control one;collected their fasting venous blood for detecting its Septin 9gene methylation by using real-time fluorescent probe PCR method;then compared Septin 9methylation levels between patients and healthly persons,and between before and after treatment.As results,in observation group 47cases were in positive expression,17cases in negative one,thus positive expression ratio was 73.4%,while in control one,respectively 0,64(cases),the ratio was 0,so there was significant difference between both groups(P <0.05);in observation group,49cases received surgical treatment in whom 37 cases were positive expression before surgery,12cases negative one,then conversing negative in 27cases, 15cases chemotherapy in whom respectively 10cases,5cases,then 1case;postoperative follow-up of 1year found 19cases suffered from recurrence or metastasis far away 19cases,in whom 14cases were in positive expression(the ratio,73.7%),7cases were in serum CEA positive expression(the ratio,36.8%),there was statistical difference between both positive ratio(P <0.05).Results show that detecting Septin 9methylation level can enhance diagnostic accuracy of colorectal carcinoma,thus Septin 9could be used as a molecular marker in the evaluation on effect and in postoperative follow-up monitoring.
