角膜塑形镜用于青少年近视进展控制的临床效果观察

Clinical effect of orthokeratology in controlling the progress of myopia in adolescents

目的观察角膜塑形镜用于青少年近视进展控制的临床效果。方法回顾性分析青少年近视患者140例(280眼)临床资料。依据是否配戴角膜塑形镜分为对照组和观察组,每组70例。对照组配戴框架眼镜,观察组配戴角膜塑形镜。观察2组戴镜前及戴镜后1、3、6、12个月时的屈光度、眼轴长度、角膜内皮细胞密度及角膜中央厚度。结果戴镜后,2组屈光度、眼轴长度改善显著,配戴12个月后,观察组屈光度、眼轴长度均好于对照组,差异均有统计学意义(P <0. 05)。戴镜后,2组内皮细胞密度及角膜中央厚度与戴镜前比较差异无统计学意义(P> 0. 05),组间比较差异亦无统计学意义(P> 0. 05)。结论应用角膜塑形镜治疗青少年近视,可有效改善其屈光度,预防眼轴增长,对角膜内皮细胞的影响较小,值得临床推广应用。

Objective To analyze the clinical effect of orthokeratoscopy in controlling the progress of myopia in adolescents. Methods A retrospective analysis was made of 140 cases( 280 eyes) of juvenile myopia admitted to the optometry Department of our hospital from January 2016 to December 2018. According to whether the orthokeratology lens can be worn,the patients were divided into control group and observation group,70 cases in each group. The patients in the observation group received orthokeratology lens,while the patients in the control group wore frame glasses. The refraction,axial length,corneal endothelial cell density and central corneal thickness were observed before and 1,3,6 and 12 months after wearing the glasses. Results After wearing glasses,the diopter and eye axis length of the two groups improved significantly. After wearing glasses for 12 months,the diopter and eye axis length of the observation group were better than those of the control group,the difference was statistically significant( P < 0. 05). After wearing glasses,endothelial cell density and central corneal thickness decreased in both groups,but there was no significant difference compared with before wearing glasses( P > 0. 05).There was no significant difference between the two groups. Conclusion The application of keratoplasty lens in the treatment of juvenile myopia can effectively improve the diopter and prevent the growth of eye axis. It has little effect on corneal endothelial cells and is worthy of clinical application.
