摘要 目的探讨3. 0 T高分辨磁共振(HR-MRI) T2加权成像(T2WI)及扩散加权成像(DWI)序列在直肠癌术前TN分期评估中的应用价值。方法我院59例直肠癌患者,均于术前进行3. 0 T HR-MRI T2WI和DWI序列扫描,以手术病理结果为金标准,分析3. 0 THR-MRIT2WI及DWI序列对直肠癌术前T、N分期评估情况。结果手术病理结果显示59例直肠癌患者T1期、T2期、T3期和T4期分别为4例、6例、45例和4例,N0期、N1期及N2期分别为35例、15例、9例。3. 0 THR-MRI T2WI及DWI扫描对直肠癌术前T分期诊断总准确度为91. 53%,其中T1期、T2期、T3期和T4期灵敏度分别为75. 00%、66. 67%、95. 56%、100. 00%,特异度分别为100. 00%、94. 34%、85. 71%、100. 00%,准确度分别为98. 31%、91. 53%、93. 22%、100. 00%。3. 0 THR-MRI T2WI及DWI扫描对直肠癌术前N分期诊断总准确度为84. 75%,其中N0期、N1期、N2期灵敏度分别为85. 71%、80. 00%、88. 89%,特异度分别为95. 83%、86. 36%、96. 00%,准确度分别为89. 83%、84. 75%、94. 92%。结论 3. 0 THR-MRI T2WI及DWI序列扫描是术前T、N分期评估重要方法。 Objective To investigate the application value of 3. 0 T high-resolution magnetic resonance imaging( HR-MRI T2-weighted imaging( T2WI) and diffusion-weighted imaging( DWI) in preoperative T and N staging of rectal cancer.Methods The clinical data of 59 patients with rectal cancer were retrospectively analyzed.All patients underwent 3. 0 T HR-MRI T2 WI and DWI sequences scan before operation.With operation and pathological results as the golden standard,the evaluation of preoperative T and N staging of rectal cancer by 3. 0 T HR-MRI T2 WI and DWI sequences was analyzed. Results The results of operation and pathology showed that there were 4,6,45 and 4 cases at T1,T2,T3 and T4 stages,respectively,and 35,15 and 9 cases at N0,N1 and N2 stages,respectively.The total accuracy of 3. 0 T HR-MRI T2 WI and DWI scan for preoperative T staging of rectal cancer was 91. 53%.The sensitivities for T1,T2,T3 and T4 stages were 75. 00%,66. 67%,95. 56% and 100. 00%,the specificities were 100. 00%,94. 34%,85. 71%and 100. 00%,and the accuracy rates were 98. 31%,91. 53%,93. 22% and 100. 00%,respectively.
