摘要
目的探讨64排螺旋CT联合应用低管电压、低管电流和低对比剂浓度行肺动脉成像的可行性。方法将90例疑似肺栓塞行肺动脉成像检查的患者分成常规剂量组、低剂量组各45例。常规剂量组扫描参数:管电压120 k V,管电流250 m As,对比剂浓度370 mg I/ml;低剂量组扫描参数:管电压100 k V,管电流200 m As,对比剂浓度300 mg I/ml。由2名主治及以上职称医师以统一标准对两组图像进行主观、客观评价。结果两组图像质量主、客评价比较差异无统计学意义(P>0. 05)。两组有效剂量ED分别为(8. 45±0. 32) m Sv、(3. 30±0. 16) m Sv,对比剂碘含量分别为11. 1 g、9. 0 g,低剂量组辐射剂量与对比剂碘含量均较常规剂量组低(P <0. 05)。结论 64排螺旋CT联合应用低管电压、低管电流和低对比剂浓度在肺动脉成像中是可行的,在图像质量满足临床诊断要求的前提下,可降低辐射剂量及对比剂碘含量。
Objective To explore the feasibility of 64-slice spiral CT combined with low-tube voltage,low-tube current and low contrast agent concentration in pulmonary artery imaging.Methods Ninety patients with suspected pulmonary embolism were divided into routine dose group and low dose group,45 in each group.The scan parameters of the routine dose group were tube voltage 120 kV,tube current 250 mAs,and contrast agent concentration 370 mgl/ml.The parameters of the low dose group were tube voltage 100 kV,tube current 200 mAs and contrast agent concentration 300 mgL/ml.The images of the two groups were evaluated subjectively and objectively by 2 experienced physicians according to equal standards.Results There was no significant difference in image quality between the two groups evaluated subjectively or objectively(P>0.05).The effective doses Ed of the two groups were 8.45±0.32 msv and 3.30±0.16 mSv,respectively.The iodine content of contrast agent was 11.1 g and 9.0 g,respectively.The differences between the two groups were statistically significant(P<0.05).Conclusion 64-slice spiral CT combined with low-tube voltage,low-tube current and low contrast agent concentration in pulmonary artery imaging is feasible.Based on the premise of satisfying the clinical diagnostic re-quirements,the radiation dose and the volume of contrast agent can be reduced.
