3M透明敷贴联合体表导管固定装置或缝合固定双腔中心静脉导管的效果观察

The clinical effects of 3M Tegaderm combined with body surface catheter fixing device or suture for fixation of central venous catheter

目的评价3M透明敷贴联合体表导管固定装置或缝合固定双腔中心静脉导管(CVC)的临床效果。方法选取我院2016年7月至2016年12月接受胃肠道手术,纳入住院期间置有双腔CVC患者200例,随机数字表法分为试验组及对照组各100例,试验组采用免缝合的导管固定装置联合3 M透明敷贴固定导管,对照组采用缝合皮肤加3 M透明敷贴固定导管。比较两组患者带管时间、导管滑脱、局部感染、换药次数、导管移位长度及患者不适感、局部疼痛等。结果两组患者带管时间、换药次数和导管移位长度比较差异无统计学意义(P> 0. 05),试验组的导管滑脱率及局部感染率均低于对照组(P <0. 05),患者自报不适感率及局部疼痛评分低于对照组,差异有统计学意义(P <0. 05)。结论与经皮缝合联合3 M比较,免缝合的导管固定装置联合3 M透明敷贴固定双腔CVC导管能有效降低脱管率、局部感染率及增加患者舒适度。

Objective To investigate the clinical effects of 3 M Tegaderm combined with body surface catheter fixing device or suture for fixation of central venous catheter( CVC). Methods We selected 200 patients who received gastrointestinal surgery with CVC in hospitalization in our hospital between Jul 2016 and Dec 2016.The patients were divided into experimental or observation group through random number table,100 in each group.The body surface catheter fixing device combined with 3 M Tegaderm were used to fix the CVC for the experimental group while suture combined with 3 M Tegaderm for the observation group.The time between the two groups of indwelling catheters,incidence of catheter slippage,local infection rate,number of dressing changes,length of catheter displacement,and discomfort and local pain of the patient’s were compared. Results There was no significant difference in the time of indwelling catheter,number of dressing changes and length of catheter displacement between the two groups( P > 0. 05).However,the catheter slip rate and local infection rate as well as sensory rate and local pain scores in the experimental group were significantly lower than those in the control group( all P < 0. 05).Conclusion The 3 M Tegaderm combined with body surface catheter fixing device has a less rate of catheter slippage and local infection and improves the comfort when compared with the 3 M Tegaderm combined with suture.
