一例关于2型糖尿病合并高脂血症患者的临床药学监护

A Case of Clinical Pharmaceutical Care for Patients with Type 2 Diabetes Mellitus Complicated with Hyperlipidemia

目的探讨2型糖尿病合并高脂血症患者的药学监护及个体化治疗方案,规范药学服务方法及模式。方法临床药师依据药物的作用特点,同时结合患者病理生理情况,为1例糖尿病高血脂症患者制定用药方案,定期监护血糖及肝肾功能,及时进行用药重整,并进行用药教育。结果通过医师和临床药师的努力,调整完善了治疗方案,患者血糖控制尚平稳,准予出院。结论糖尿病患者免疫力低下,且合并高脂血症,临床药师因根据患者情况个体化用药并全面监护,可使患者血糖、血脂控制平稳,促进临床合理用药。

Objective To investigate the pharmacological monitoring and individualized treatment of patients with type 2 diabetes mellitus complicated with hyperlipidemia,and to standardize the methods and modes of pharmaceutical services.Methods Based on the characteristics of the drug and combined with the pathophysiology of the patient,the clinical pharmacist developed a drug regimen for a patient with diabetes mellitus and hyperlipidemia,regularly monitored blood glucose and liver and kidney functions,timely reconstituted the drug,and conducted medication education.Results Through the efforts of physicians and clinical pharmacists,the treatment plan was adjusted and improved,and the patient's blood glucose control was still stable and was allowed to be discharged.Conclusion Diabetic patients have low immunity and hyperlipidemia.The clinical pharmacists can control the blood glucose and blood lipids smoothly and promote clinical rational drug use because of individualized medication and comprehensive monitoring according to the patient's condition.
